Box 1179 Southold, New York 11971 Re: Proposed Wireless Communications Facility Collocation Cellco Partnership and its controlled affiliates d/b/a Verizon Wireless 1950 Breakwater Road Mattituck, New York 11952 Suffolk County SCTM# District 1000—Section 106—Block 9—Lot 6.001 Dear Ms. Lanza: VHB Engineering, Surveying and Landscape Architecture, P.C.(VHB) is serving as environmental consultant to the applicant referenced above for the purpose of obtaining clearance from the New York State Office of Parks, Recreation and Historic Preservation (OPRHP), pursuant to the Code of Federal Regulations (CFR) Title 47, Chapter I, Part I, Subpart I, Section 1.1307 (a)(4).In accordance with the Nationwide Programmatic Agreement,' notice is hereby given that Verizon Wireless is proposing the installation of a wireless communications facility within and adjacent to the 35-foot-4-inch above grade level (agl) (with appurtenances) Transfiguration of Christ Greek Orthodox Church building located at 1950 Breakwater Road, Mattituck, New York, and VHB is requesting comments regarding potential effects upon historic resources associated with the proposed action. Specifically, Verizon Wireless is proposing the installation of six panel antennas within a proposed radio- frequency transparent extension to the existing church cupola,which would match the existing cupola visually.The overall height of the proposed cupola extension would be 46-feet agl upon completion.In addition, twelve remote radio head (RRH) units,three main distribution boxes (MDB) and associated appurtenances would be installed within the lower portion of the existing cupola.The proposed installation would also include a 10 kilowatt natural gas emergency backup generator and ancillary equipment upon a concrete pad,within a 154 ±-square-foot fenced outdoor equipment area,which would be located adjacent to the western wall of the subject building.Associated cables would be routed within the subject building to the proposed antennas. Finally,telco, and electric lines would be routed overhead from a nearby utility pole. 'The Nationwide Programmatic Agreement was promulgated by the Federal 100 Motor Parkway communications Commission establishing guidelines for review of proposed wireless Suite 135 communications facilities and possible impacts on historic properties.
