1. Final survey of property with accurate location of all buildings, properW lines, streets, end unusual
natural or topographic featu res.
2.Final approval of Health Dept. of water supply and sewerage disposal-{S-9 form or equal).
3.Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings. Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5.Submit Planning Board approval of completed site plan requirements where applicable.

Dear Tom:
The following should clarify your questions con-
cerning the location of the above dwelling,
relative to the Flood Plain:
1. The property is partially in Flood Zones A 7, B
and C. The demarcation line between A 7 and B oc-
curs at the contour +8. You will note on the at-
tached survey that contour +8 is located on the
westerly most portion of the property, near the
bulkhead.
2. In overlaying the boundaries of the property on
the Flood Plain Map, Panel 0058D, dated April 17,
1985, it is evident that all proposed construction
fall in Zone B, which is above the highest reaches
of potential flood waters.
It is therefore my opinion that the proposed dwell-
ing will be constructed in compliance with all as-
pects of the Flood Plain ordinance, as depicted on
the drawings on file in your office.
If you have any further questions, please feel free

DESCRIPTION DE AUTHORIZED ACTIVITY
Construct a single family dwelling with decking all parts of which are to be a
minimum of 50' landward of bulkhead. Install septic system to east of house; install
dry-wells to contain roof run off; construct sauna or hot tub within area of decking
to drain into dry well. Work to be in accordance with attached p~an approved by NYS!
GENERAL CONDITIONS
By acceptance of this permit, the permtltee agrees that the permit is contingent upon strict compli-
ance wilh the ECL, all applicable regulations and the conditions specified herein or attached hereto·
The permittee shalJ file m the office of the appropriate regional permit admimstrator, or other office designated in the special conditmns, a notice of intention to commence
work at least 48 hours in advance of the time of commencement and shall also notdy him/her promptly in writing of the completion of the work.
2 The permitted work shall be subject to inspection by an authorized representative of the Departmeqt of E~wiroRmenta[ CoRse~vaC~on which may o~det the work suspended if
the public interest so requires pursuant to ECL §73-0301 and 5APA §401(3)
3 The permittee has accepted expressly, by the execution of the apphcation, the full legal responsibility for all damages, direct or redirect, of whatever nature, and by whomever
suffered, arising out of the project described herein and has agreed to indemnify and save harmless the State from suits, actions, damages and costs of every name and descrip-
tion resulting from the said project.
4. The Department reserves the right to modify, suspend or revoke this permtt at any time after due nobce, and. if requested, hold a hearmg when.
a) the scope of the project is exceeded or a violation of any condition of the permit or provisions of the ECL and pertinent ~egulations are found, or
b) the permit was obtained by m~srepresentation or failure to disclose relevent facts; or
c) newly discovered information or significant physical char~ge~ are discovered since the permit was issued,
5 The permittee is responsible for keeping the permit active by submitting a renewal appiica, tio~, inc~udmg any {otms, fee~ or supplementat information which may be required
by the Department, no later than 30 days (180 days for SPDES or Sohd or Hazarduous Waste Management permits) prtor to the expiration date.
6. This permit shaft not be construed as conveyinB to the applicant any right to trespass upon the lands or interfere with the riparian rights of others in order to perform the
permitted work or as authorizing the impairment of any rights, title or interest in real or personal property held or vested in a person not a party to the permit.

Any material dredged in the prosecution of the work herein permitted
shall be removed evenly, without leaving large refuse pdes, ridges across
the bed of a waterway or floodplain or deep holes that may have a
tendency to cause damage to navigable channels or to the banks of
a waterway.
There shall be no unreasonable interference with navigation by the work
heresn authorized.
if upon the expiration or revocation of this permit, the project hereby
authorized has not been completed, the applicant shall, without expense
to the State, and to such extent and in such time and manner as the
Oepartment of Environmental Conservation may require, remove all or
any portion of the uncompleted structure or fill and restore the site
to its former condition, No claim shall be made against the State of

Dear Mr. Strang:
As per your letter of January 21, 1986 requesting a
determination on the need of a Wetland Permit for the above-
referenced, please be advised that the Trustees have reviewed
your site plan submitted and inspected the premises on Snug Harbor
Road and Maple Lane, Greenport.
The Trustees have determined that no permit will be needed
for the construction as proposed on the site plan submitted, as
all construction is behind an existing bulkhead. The Trustees
would like to you take the following measures during construction:
1. Roof run off shall be discharged into dry wells.
2.' Driveways and parking areas within 75' of the water
shall be of porous construction. Blacktop shall not
be used on this site.
3. Care shall be taken so as to preclude the entry of
any construction material into adjacent waters.
4. Immediately following construction, all areas of
exposed soil shall be properly protected with
suitable erosion control devices and/or vegetated
to prevent entry of eroded soil into nearby waters.

WHEREAS, at said hearing all those who desired to be heard
were heard, and their testimony was recorded; and

and documentation submitted concerning this application; and
WHEREAS, the board made the following findings of fact:

of Maple Lane and the north side of Snug Harbor Road, Greenport,

June 14, 1966 as File No. 4650.
2. The subject premises consists of a total area of
approximately .35 of an acre with frontage along Maple Lane of
50 feet, along Snug Harbor Road of 103.40 feet, and along Dawn
