APPLICATION FOR CERTIFICATE OF OCCUPANCY
This application must be filled in by typewriter OR ink and submmtted to the buildzng
inspector with the followzng: for new bu~ldzng or new usa:
1. Fznal survey of property with accurate locatmon of all buzldings, property lines,
streets, and unusual natural or topographzc features.
2. Final Approval from Health Dept. of water supply and sewerage-dzsposai(S-9 form).
3. Approval of electrical installation from Board of F~re Underwrmters.
4. Sworn statement from plumber certifying that the solder used in system contazns
less than 2/10 of 1% lead.
5. Commercial building, industrial bumlding, multiple residences and similar buildings
and installations, a certificate of Code Complzance from architect or engineer
responsible for the building.
6. Submzt Plannzng Board Approval of completed smte plan requzrements.
For existzng buildings (prmor to April 9, 1957) non-conform~ng uses, or buildings and
"pre-exmstzng" land uses:
i. Accurate survey of property showing all property lznes, streets, buzldzng and
unusual natural or topographic features.
2. A properly completed applzcatzon and a eonsant to inspect signed by the applicant.
If a Certifzcate of Occupancy ms denied, the Building Inspector shall state the
reasons therefor in wrzting to the applmcant.

On 7/19/88 an inspection was done and the flue pipe passed
inspection. No one has called for a final inspection and no
Certzficate of Occupancy has been issued for the work.
If the work is finished please call our offzce to arrange
for the final inspection. If everything passes then file for
the Certificate of Occupancy.
