This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial building,multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses,or buildings and "pre-existing"land uses:
1. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is
denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

Dear Gary,
The walls and ceiling of the garage in the above referenced residence were insulated
with R-19 fiberglass batts prior to the installation of gypsum wall board. This may have
been noticed by the inspector who performed the insulation inspection. The windows
have insulating glass. I hereby certify that the space meets the requirements of the
New York State Energy Code, as does the entire house.
Please note that the heat in the garage is intended only to keep this space from
freezing; it will not be used to maintain a temperature suitable for an occupied space.

Dear Bruno,
In response to your concern, please note the following:
CONSTRUCTION CLASSIFICATION
The construction classification for the new and existing structure is Type 5a, as per
TABLE 1-704: i.e., all structural elements are protected with fire-resistive materials of at
least 3/4 hours. The existing plaster walls and ceilings have a rating of one hour, as per
Design # U315 of the 2000 Fire Resistive Directory, and the new walls and ceilings
(constructed with 5/8" Type "X" gypsum board) also have a rating of one hour, as per
Design #U305 of the same manual. The roof framing is not required to be rated, as per
Footnote 2 of TABLE 1-704. With the classification of Type 5a, the permitted fire area is
7000 square feet.
THIRD FLOOR
The existing third floor, which has been included as habitable space in the recently
issued Certificate of Occupancy, will not be altered without necessary approvals.
FIRE AREA
The fire area, within exterior walls, of the new and existing structure on all three
habitable levels equals 6,915 square feet, below the maximum 7000 square feet.
Please call with any questions. Otherwise, please complete your review and issue a
Building Permit.

Based on the information you have submitted, the New York State
Department of Environmental Conservation has determined that :
The property landward of the bulkhead greater than 100 ' in length,
constructed prior to 8/20/77 as shown on the tidal wetland aerial
photomap #714-544 and confirmed on IR photo # 820 , is beyond the
jurisdiction of Article 25 (Tidal Wetlands) .
Therefore, in accordance with the current Tidal Wetlands Land Use
Regulations (6NYCRR Part 661) no permit is required under the Tidal
Wetlands Act . Please be advised, however, that no construction,
sedimentation, or disturbance of any kind may take place seaward of the
tidal wetlands jurisdictional boundary, as indicated above, without a
permit . It is your responsibility to ensure that all necessary
precautions are taken to prevent any sedimentation or other alteration or
disturbance to the ground surface or vegetation within Tidal Wetlands
jurisdiction which may result from your project . Such precautions may
include maintaining adequate work area between the tidal wetland
jurisdictional boundary and your project (i .e . a 15 ' to 20 ' wide
construction area) or erecting a temporary fence, barrier, or hay bale
berm.
Please be further advised that this letter does re ve you of the
