APPLICATION FOR CERTIFICATE OF OCCUPANCY
This application must be filled in by typewriter OR ink and submitted to the building
inspector with the following: for new building or new use: "
1. Final survey of property with accurate location of all buildings, property lines,
streets, and unusual natural or topographic features.
2. Final APproval from Health Dept. of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains
less than 2~10 of 1% lead.~
5. Commercial building, industrial building, multiple residences and similar buildings
and installations, a certificate of Code Compliance from architect or engineer
responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
For existing buildings (prior to April 9, 1957) non-~onforming uses, or buildings and
"pre-existing" land uses:
i. Accurate survey of property showing all property lines, streets, buildimg and
unusual natural or topographic features.
2. A properly completed application and a consent to inspect signed by the applicant'.

Enclosed please find the title certification of the above
referenced property. Mr. Hiltz is purchasing two lots from Edna
McNulty and John R. McNulty. The properties are lots on & file4 map
of "Moose Cove". The lots are exempt from the merger law because
the title is single & separate from 1/1/97.
These properties should be entitled to building permits
without the expense of a single & separate title search. We have,
as purchasers, obtained a title report for each property which
provides the title certification you need.
Enclosed is the title certification page from both lot
purchases: SCTM 1000-103-04-Lot 6 is owned by Edna V. McNulty and
John R. McNulty who acquired title by deed dated 12/27/96 (to avoid
merger of title) and 1000-97-07-Lot 19.003 is owned by Edna V.
McNulty since 1969.
