Kassner, Site Plan Reviewer SUBJECT: Daval Health Department Approval DATE: May 5, 1995 On May 3, 1995, I called Mr. Robert Farmer at the Suffolk County Health Services Board of Review to discuss the Donald Grim property. In the course of conversation Mr. Farmer mentioned other properties in the area were also having problems with water purity. He mentioned specifically Daval as now having a problem with their water. The former letter indicates that there is degraded water quality at the above-noted site. However, since the SCDHS does not prohibit the use of the site, it would appear that this situation does not prevent the issuance of a Certificate of Occupancy. The second letter indicates the bank's agreement to see to the installation of the required curbing at the time of transfer of ownership. This letter complies with our request of 1992 to SCNB. Accordingly, this Board has no objections to the issuance of a CO by the Building Department at this time. Heidtmann's request of last week, this site was inspected on July 12, 1990. The site has not been completed in accordance with the site plan. The following matters need to be resolved: 1. There was standing water around the center roof drair. pipe where it entered the ground. There was also a long, eroded section in the lawn between the standing water and a low, wooded area between the building and the street. Could the drywell have failed? 2. The sign at the entrance has not been installed. 3. Curbing has not been installed anywhere in the parking area or the entrance drive into the parking area. 4. There are 11 parking spaces along the northern end of the parking field. The approved plan shows 13 spaces. However, there are-23 marked spaces along the western end of the parking area, and only 7 are called for on the site plan. Not all of these spaces are usable. See note 5. below. Also, an additional 8 spaces have been installed along the southern end of the parking field. This space was marked for outside storage on the site plan. In general there are more spaces than required by the plan. This is not seen as a problem. Although, an amended plan should be filed showing the parking as it • has been installed. 5. Several of the parking spaces along the southwesterly end of the parking field were occupied by 14 "sanitary outhouses". A well and hose have been installed in front of about two of the spaces, which prohibits their use as parking spaces. It appears that the porta-potties are emptied here into a scavenger waste receptacle mounted on a small flatbed truck that was parked nearby. The hose appears to be used for washing down the structures. This use was not shown on the approved site plan. It is not known if • Health Department approval may be required for such a washdown area. 6. The parking area needs to be resurfaced. Grass is growing through the asphalt. 7. A large underground„gas (propane?) storage tank has been installed along the southerly property line. This tank was not shown on the approved site plan. It is not known whether this tank has permits from the Building Department. A smaller propane tank was found at the rear of the building next to the loading ramp. 8. The rear or westerly slope abutting the parking lot shows signs of erosion. The erosion is particularly severe at the northwest corner of the parking field. A landscape plan was not submitted as per the Board's letter of December 6, 1989. The slopes have been planted with grass. Slopes appear to be on the steep side for mowing. 9. The dumpster is located along the southerly part of the parking field, not as shown on the site plan. Also, it is not enclosed by a stockade fence. 10. The six-foot stockade fence has not been installed at the south end of the parking field. 11. A total of about nineteen 50-gallon drums are being stored in the parking field. Is this storage legal? What is in them? Five of the drums are in the parking spaces at the entrance. These drums were not empty. Fourteen empty drums are under a trailer flatbed that is parked opposite the entrance, in the northwest corner of the parking field. The trailer has an 1987 sticker on it and has steps leading to it. Is this type of storage legal? Is it permanent? '~r ~ ~ Because of the large number of outstanding items that either were not completed in accordance with the site plan or that were not shown on the approved site plan, the issuance of a Certificate of Occupancy is not recommended. Upon resolution of the above-noted items, please notify this office to schedule another inspection.
