This application must be filled in by typewriter OR ink and submitted to the building
inspector with the following: for new building or new use:
1. Final survey of .property with accurate location of all buildings, property lines,
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form) .
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings
and installations, a certificate of Code Compliance from architect or engineer
responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and
"pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and
unusual natural or topographic features.
2. A properly completed application and a consent to inspect signed by the applicant.
If a Certificate of Occupancy is denied, the Building Inspector shall state the
reasons therefor in writing to the applicant.

Dear Sir:
The above referenced project was inspected on November 5, 1998,at the request of Mr.
Fred Milner. The purpose of the inspection was to determine if a cathedral ceiling could be
provided on the second floor of the structure in the dormer area.
After analysis of the as-built conditions at this stage of the project,the following is
proposed to provide the requested cathedral ceiling:
• In lieu of the planned ceiling joists, install under the existing ridge beam a structural ridge,
consisting of a 5 '/2"x14"Anthony Power Beam LSL as manufactured by Anthony Forest
Products Company.
• On the second floor,directly over the two center interior columns supporting the LVL floor
girder,post the structural ridge beam using 5 'h"x5 %2"timber posts and solid block to the
LVL girder below. At the gable ends,use the equivalent to(3)2"x6"built-up posts.
• At first level, remove the two existing center interior columns and replace with 3'h"standard
steel pipe columns with 1/4"steel plates top and bottom.
• Use approved hurricane ties to anchor the roof rafters to the top plate at the cathedral area.
It is my professional opinion that this configuration provides for adequate support of the
anticipated loads of the region and is in conformance with the general intent of the New York
State Building Construction Code.
