A. This application must be filled in by typewriter OR ink and submitted to the building
inspector with the following: for new building or new use:
L. Final survey of .property with accurate location of all buildings, property lines,
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of 17, lead.
5. Commercial building, industrial building, multiple residences and similar buildings
and installations, a certificate of Code Compliance from architect or engineer
responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.

Dear SirlMadaln:
I would like to meet with a Building Inspector to review the various certificates of
pre-existing use and certificates of occupancy heretofore issued in respect to these
premises. At present there is a contract of sale, and the purchaser wishes to be sure the
premises heating system is approved by the Town.
For convenience, I enclose:
1. Certificate ofPre-Existing Use issued on July 6, 1988 (The Kars took title
on August 4, 1988).
2. Certificates of Occupancy 219800 for Building Permit 175692 of
October 28, 1988 issued March 18, 1991, and referencing Plumbers
Certification of Liso Plumbing of March 13, 1991.
3. Certificate of Occupancy 219801, also dated March 18, 1991, for Building
Permit 191242 of June 12, 1990.

4. Certificate of Occupancy Z-20330 dated November 7, 1991 for Building
Permit 18868-Z of March 13, 1990, referencing Plumbers Certification of
Walter L. Dohm of November 1, 1991.
Underwriters Certificate No. 158435 of October 31, 1990, referred to in
number "4" above, and containing a reference to one gas furnace.
(This was apparently received by your department on February 6, 1991,
prior to the issuance of number " 2" above and at the same time as the
underwriters certificate referenced in " 2" above).
The client's file, which I will bring, contains a January 24, 1991 inspection report
by Inspector Gary J. Fish which reflects the existence of a heating system and a request
that the "... heating contractor cafl Building Department."
Lastly I enclose the November 23, 1988 proposal of Liso & Dooley Plumbing to
the Kars, which includes: "...move baseboard heat and add additional radiation in new
section. Any gas piping will be extra."
I hope these enclosures will aid your Department's review of your file for these
premises and expedite our meeting and your Department's suggestions. I would prefer a
meeting on a Tuesday or Thursday as soon as is convenient. ,,

INSTRUCTIONS
a. Tltis application must be completely filled in by typewriter or in ink and submitted to the Building Inspector, with 3
sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan shoving location of lot and of buildings on premises, relationship to adjoining premises or public streets
or areas, and giving a detailed description of layout of property must be drawn on the diagram which is part of this appli-
cation.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building inspector will issued a Building Permit to the applicant. Such permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose whatever until a Certificate of Occupancy
shall have been granted by the Building Inspector.
