APPLICATION FOR CERTIFICATE OF OCCUPANCY
A. This application must be filled in by typewriter OR ink and submitted to the building
inspector with tho following: for new building or new use:
1. Final survey of property with accurate location of all buildings, property lines,
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board'of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings
and installations, a certificate of Code Compliance from architect or engineer
responsible for the building. '
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9 1957) non-conforming uses, or buildings and
pre-existing" land uses:
I.Accurate survey of property showing all property lines, streets, building and
unusual natural or topographic features.
2. A properly completed application and a consent to inspect signed by the applicant.
If a Certificate of Occupancy is denied, the Building Inspector shall state the
reasons therefor in writing to the applicant.

Installation Instructions
I CAUTION: Make certain existing wallboard is fastened
securely to stud wall before fastening wall shield,

Take your wall shield and measure in from each side near
corners approximately 6", and approximately 1 2',', from
L//' sides for center holes. Note Illustration A. Use 3/16 steel
bit and drill hole through wall shield. Then use 1/2" steel
bit and very, very carefully drill recessed hole 3/8" deep
directly in hole just drilled. Note Illustration C.

Place waif shield against wall in desired location, resting on
floor, Then using a small pointed object, insert it through
holes in wall shield marking each location on wall Then
remove wall shield and drill 5/8" hole through wall board at
each marked location.

Position wall shield back in place. Then, leaning wall shield
forward, insert 3/16" toggle bolts through holes in wall
shield and screw toggles on ends of bolts. Then starting at
bottom, guide toggles through holes in wall. Note Illustra-
tion B, Then tighten snugly with screwdriver. Note Illustra-
tion D.

Make sure all toggle bolts are
tightened moderately. Then using
touch-up material provided with wall
shield fastening kit, fill each hole
evenly with putty knife or finger.

Any other type of securing device
must be countersunk and covered
by an insulating material such as
KemStone putty.

The KEMSTONE FLOOR PROTECTOR is designed for use
under solid fuel type heaters and fireplace stoves and is in-
tended to be used in place of a 3/8 in. non-combustible in-
organic millboard equivalent to a millboard having a thermal
'conductivity - K=0.21 btu=in/fi 2.h,' F".
The KemStone floor protector is intended only for use with
UL and ULC listed solid fuel type room heaters and listed
fireplace stoves listed in accordance to UL1482 and
ULC627 standards.
Floor protectors must be installed on a flat solid surface and
any masonry or metal cutting device may be used if it is
necessary to cut.

CONNECTOR INSTRUCTIONS
When joining 2 or more KemStone boards together,
KemStone connector mouldings must be used - See II1. C

Lay floor protector A and floor protector B in place, leaving
enough space to sllp connector molding onto either one of
the floor protectors A and B. Be sure to install the connec-
tor molding with the insulated rope side up, as in illustration
C. Then, after connector molding is in place, slide floor pro-
tectors A and B tightly together,
INSTALLER
Please check with the Fireplace Stove er Room Heater's In-
stallation instructions for minimum area required for the floor
protector.
WARNING
DO NOT SLIDE STOVE ON FLOOR PROTECTOR. If stove'
legs are sharp or pointed, we recommend using a non-
combustible caster cup. '

a. This application must be completely filled in by typewriter or in ink and submitted to the Building Inspector, with 3
sets of plans, accurate plot plan to scale· Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets
· or areas, and giving a detailed description of lhyout of property must be drawn on the diagram which is part of this appli-
cation.
c. The work covered by tkis application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issued a Building Permit to the applicant. Such permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose whatever until a Certificate of Occupancy
shall have been granted by the Building Inspector.
