A. This application must lie filed in typewriter OR ink and submitted Co the Building
Inspector with the folliowing; for new buildings or new use:
1. Final survey of property with accurate location of all buildiogs, property lines,
streets, and uausualj natural or topographic features. '
2. Final approval of Dgalth Dept. of water supply and sewerage-disposal~S-9 form),
' 3. Approval of clectrigal installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying,thaC solder used in system contains less"

6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings'CPrior to April 9, 1957 non-conforming uses, or buildings
and "pre-existing" land uses:
I. Accurate survey of property showing all property lines, streets, buildings and
unusual natural or topographic features,
2. A properly completed application, a consent to inspect signed by the applicanC
and a certified abstract of title issued by a title company which shall
show single and separate ownership of the entire lot prior to April 9, 1957.
If a Certificate of~'Occupancy is denied, the Building Inspector shall state the
reasons Cherefor in ',writing to the applicant.
3. Date of any housinglcode or safety inspection of buildings or premises, or other
pertinenC information required to prepare a certificate.
C. For Vacant Land Certificate of Occupancy:
1. An application for vacant land Certificate of Occupancy shall be submitted,
and a certified abs~ract of title issued by a title company showing single and
separate ownership pf~the entire lot prior to April 9, 1957 shall also accompany
the application. If a Certificate of Occupancy is denied, the Building Inspector
shall state the reasons therefor in writing to the applicant.

SUBJECT: 2nd DWELLING ON PROPERTY OF EUGENIA (RAPUSTKA) BEEBE
DATE: DECEMBER 1, 1989
Property has had two dwellings long before Zoning went into
effect. Both Dwellings have always been used (uninterupted)
since Zoning. The dwelling in question had an application to
put a foundation under it. In process of doing same, it was
discovered that termites and rot had destroyed a large part of
the sills and floor timbers. Permit fee was adjusted to replace
same, and later amended for addition.
This property is situated in the A-Residential Zone. The
New York State Education Law 7202 etc. would not be involved
here as it would be mandatory in all but residential. Which
structure would be the non-conforming one is not answerable, but
believe this would not have any bearing anyway. At present I
cannot see anything that is not allowable by law. Permit is
still active and no C.O. has been issued. A copy of permit and
drawing enclosed.

sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plat plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets
or areas, and giving a detailed description of layout of property must be drawn on the diagram which is part of this appli-
cation.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approvai of this application, the Building Inspector will issued a Building Permit to the applicant. Such permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose whatever until a Certificate of Occupancy
shall have been granted by the Building Inspector.
