Instructions
A. This application must be filled in typewriter OR ink, and submitted a ~ to the Building Inspec-
torwith the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, acertificate of Code compliance from the Architect or Engineer responsible for the, building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
` 1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or

Dear Mr. Power:
Thank you for the cooperation during the fire inspection of
the Sea Shell Restaurant on August 25, 1988.
Durinq the inspection the following were noted and should be
corrected as soon as possible.
' A second exit is required. The exit through the kitchen is
no longer usable, by the public, due to changes in the fire
code. There shall be an illuminated exit sign over the back
door. The door shall be equipped with emergency egress
hardware that will not impede exit at any time. Solid bars,
bolts, locks, or.any hardware that stops egress through the
door at any time is illegal and not permitted. *
All"exit signs must be illuminated any time that the
restaurant•is open to the public. The front exit is out.
There'shall be a minimum of one (1) set of emergency exit
lighting positioned so as to illuminate the exit paths in
case of power interruption.
The extinguisher behind the bar should be mounted instead of
standing on the floor. * This extinguisher is out of date
and should be properly inspected.
' The items marked with an asterisk are items noted for
correction in your last inspection, dated July 7, 1987, which
have not been corrected as of the date of this inspection.
The front door sticks at the bottom when exiting, the exit
door should operate freely at all times.
The required second exit may be through the back wall or
through a fire rated vestibule in the corner of the kitchen.
A building permit will be required for the construction.
Thls modification is necessary to maintain the occupancy load
above 49.

An exit door through the back wall shall be 49 inches wide
and be equipped with the proper emergency exiting hardware.
An illuminated exit sign shall be placed over the door and
' exit paths shall be kept clear at all times.
The vestibule concept would be a minimum of a two hour fire
rated exit area, separating the kitchen. The fire rating
would include the walls and ceiling. The existing swing door
would be removed and proper exiting hardware added to the
existing back door. Afire door could enter into the kitchen
as long as the door remained closed. Proper exit signs would
be necessary along with exit paths that remain clear at all
times.
