1. Final survey of property with accurate location of all buildings,property lines,streets,and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1%lead.
5. Commercial building,industrial building,multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings(prior to April 9,1957)non-conforming uses,or buildings and"pre-existing"land uses:
1. Accurate survey of property showing all property lines,streets,building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant.If a Certificate of Occupancy is

Dear Mr. Jarski:
As per your inspection on June 14, 2019, enclosed you will find three copies of the requested
building floor plans stamped by engineer,Nick Mazzaferro, for building#1 and building#2. All
foam insulation has been covered with 3/4"plywood, smoke and carbon oxide detectors have been
installed as well as fire extinguishers in the locations as indicated on the floor plans.
Please note that the Boatyard encompasses two adjoining lots. The buildings that I am seeking
pre-existing COs are located on the above-referenced lot.
On the other lot(SCTM#1000-64-3-11, 1000 Terry Rd)there was only one building which was
destroyed by fire and is currently being reconstructed.

each is $650 for a total of $1300. Also the property is in a flood zone AE-6 so we will need flood

I ceived 5 CO's but one building is missing. So I'm just following up to make sure that is being processed as
ell. It's building#3 on the survey. We refer to it as the "Watershed". It's an old 200 square foot masonary
building right on the road that provides access to the water mains and it used to house the coal furnace (pice
