APPLICATION FOR CERTIFICATE OP OCCUPANCY
1. This application must be filled in by typewriter OP, ink and submitted to the building
inspector with the following: for new building or new use:
1. Final survey of property with accurate location of all buildings, property lines,
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Pire Underwriters.
4, Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of 17 lead.
5. Commercial building, industrial building, multiple residences and similar buildings
and installations, a certificate of Code Compliance from architect or engineer
responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
i. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and
"pre-existing" land uses:
1. ,lc curate survey of property showing all property lines, streets, building and
unusual natural or topographic features.
2. A properly completed application and a consent to inspect signed by the applicant.
IE a Certificate of Occupancy is denied, the Building Inspector shall state the
reasons therefor in writing to the applicant.

Dear Ed:
I have enclosed,an Application for Certificate of Occupancy for an open
building permit (No.~18662Z) relating to premises at Reservoir Road on Fishers Island now
owned by William B. May, Jr. and Jeanne A. May but about to be sold at a closing
tentatively scheduled for Wednesday, November 24. (Following a lot line change in 1993,
the tax map designation of the subject property was changed from 1000-9-9-6.1 to 1000-9-
9-29.4.) I have also enclosed a check to the Town in the amount of the $25.00 application
fee, a copy of the expired permit and a copy of the existing CO for the house.
Please arrange for Bob Wall to conduct an inspection of the property as soon
as possible. We do not need the CO itself for our closing but I would like to have Bob's
oral report by Monday or Tuesday of next week. If you believe that will not be possible,
I would appreciate it if someone from your office would so advise me at your earliest
convenience.
