Dear Mr. Blasko:
In reviewing our files it has come to our attention that
there is an outstanding Order to Remedy Violations on your
property. The violations involves a deck that was constructed
around your inground swimming pool.
According to the Order to Remedy that was issued on June
20, 1990 you were required to obtain a building permit. You
have not complied with that order.
Please contact this office as soon as possible so this
matter can be resolved and avoid possible legal action.

APPLICATION FOR CERTIFICATE OF OCCUPANCY INSTRUCTIONS
This application must be filed in typewriter OR ink and submitted to the Building
Inspector with the following; for new buildings or new use:
]. Final snrvey of property with accurate location of all buildings, property lines,
streets, and unusual natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage-disposal(S-g form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that solder used im system contains less
than 2/10 of ]Z lead.
5. Commercial buildings, industrial buildings, multiple residences and similar
buildings and installations, a certificate of code compliance from the Architect
or Engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requiremeffts.
For e~isting buildings~;.(Prinr to April 9, {9573 non-conforming uses, or buildings
and "pre-existing" land uses:
{. Accurate survey of property showing all property lines, streets, buildings and
unusual natural or topographic features.
2. A properly completed~application, a consent to inspect signed by the applicant
and a certified abstract of title issued by a title company which shall
show single and separate ownership of the entire lot prior to April 9, 1957.
If a Certificate of Occupancy is denied, the Building Inspector shall state the
reasons therefor in writing to the applicant.
3. Date of any housing code or safety inspection of buildings or premises, or other
pertinent information required to prepare a certificate.
For Vacant Land Certificate of Occupancy:
]. An appllcation for vacant land Certificate of Occupancy shall be submitted,
and a certified abstract of title issued by a title eompamy showing single and
separate ownership of the entire lot prior to April 9, 1957 shall also accompany
the application. If a Certificate of Occupancy is denied, the Building Inspector
shall state the reasons therefor in writing to the applicant.

Building Permit ~18873-Z has been written for the deck
around pool. We have your check ~2730 in the amount of $55.00.
The total amount of the fee for the deck is $156.70. Please send
us your check in the amount of $131.70 representing the balance
due on the permit. We will then be able to issue the Certificate
of Occupancy for the deck.
