APPLICATION FOR CERTIFICATE OF OCCUPANCY
This application must be filled in by typewriter OR ink and submitted to the building
inspector with the following: for new building or new use:
1. Final survey of property with accurate location of all buildings, property lines,
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar..buildings
and installations, a certificate of Code Compliance from architect or engineer
responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.~
For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and
"pre-existing" land uses:
i. Accurate survey of property showing all property lines, streets, building and
unusual natural or topographic features.
2. A properly completed application and a consent to inspect signed by the applicant.
If a Certificate of Occupancy is denied, the Building Inspector shall state the
reasons therefor in writing to the applicant.

Alterations to dwelling $25.00, Swimming pool $25.00, Accessory building $25.00,

Mr. Horton:
With reference to the above Building Permit, we submit revised
plans showing the following:
1. Extra 6'0" wide by 5'0" high plate glass windows
installed on left and right side of building.
2. New steel door installed in rear. Remove existing
door and fill in wall.
3. All exterior walls to be insulated with R-19 and
gypsum board installed on the walls.
4. New hung ceiling with lay in tiles. Roof to have
insulation R-19 minimum installed.
5, Replace existing exterior wall where non-conforming
to code.
6. New partition around bathroom.
Mr. Jesse Jackson, owner of the premises is repairing the
building to make ib attractive for leasing purposes. At the
present time, three individual persons are interested in
leasing the building for retail uae.

Should the occupancy not conform to retail use, the leasee
~rould be required to file the necessary plans and applications
to the proper authorities for approval.
The "rumor" that you heard about is just that, a "rumor".
No leases have been signed by the o%~ner as of this date.
Therefore, we respectfully request that the "Stop Work
Order" be removed so %hat the balance of construction can be
completed.
