APPLICATION FOR CERTIFICATE OF OCCUPANCY
This application must be filled in by typewriter OR ink and submitted to the building
inspector with the following: for new building or new use:
1. Final survey of.property with accurate location of all buildings, property lines,
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water, supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings
and installations, a certificate of Code Compliance from architect or engineer
responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and
'7pre-existing" land uses:
I. Accurate survey of property showing all property lines, streets, building and
unusual natural or topographic features.
2. A properly completed application and a consent to inspect signed by the applicant.
If a Certificate of Occupancy is denied, the Building Inspector shall state the
reasons therefor in writing to the applicant.

Structural Inspection
Dear Mr. & Mrs. Polyn:
On November 30, 1993 my firm performed a re-inspection of your
residence under construction at the above address.
An initial inspection was performed by this firm
~-, 1990. This inspection revealed defects in construction
related to structural stability and poor workmanship.
The current inspection confirmed that all defects observed during
the first inspection were corrected by the contractor. These
corrections included the leveling of the floors and reinforcing
the structural support system for the residence's configuration.

Dear John:
Enclosed is a copy of a letter I sent to Mr. & Mrs. John
Polyn regarding the final inspection I did on there house May
10, 1991. As you can readily see by the content of the letter I
have requested that a letter be submitted to the Building
Department by a licensed architect or engineer. The letter must
address the recommendations requested by William L. Jaeger II
P.E. , L.S.
I have also contacted Richard Larsen and informed him that
there is quite a large discrepancy between the contract price
and the amount he put on his Building Permit application. He
has been informed that a stamped set of plans are required for
the job, being that it exceeded $20,000. His contract, which
Mrs. Polyn provided me with a copy, reads over $30,000.00 ~orth
of renovation. I would appreciate any type of assistance you
could provide me with regarding this matter.
If there is any way I can help you further, please feel
free to call me at 765-1802. It is best to call between 3:30
and 4:30 in the afternoon as I am on the road doing inspections
most of the day.

Dear Mr. & Mrs. Polyn:
After conducting a final inspection on May 10, 1991 and
careful consideration a Certificate of Occupancy can not be
issued for Building Permit #19055 until a number of conditions
are resolved.
Firstly, there is the outstanding matter of an engineers
report, signed by a William L. Jaeger II P.E., L.S. dated
November 8, 1990. In this report he makes a number of
recomendations. These recomendations are made to correct
problems that he sees as both structural and cosmetic in
nature. Before a CO can be issued these recomendations must be
taken care of and a letter from either the same licensed
engineer or a similar qualified engineer or architect must be
forwarded to the Building Department.
There are also a few conditions that exist that are not
mentioned in the engineers report. One of them being the fact
that the 3" waste line that was temporarily installed has a Y
coming down from the toilet fixture and flowing in the direction
of the cleanout. It is apparent that it was installed
improperly with the waste water flowing in the wrong direction.
It was stated by the contractor, a Mr. Richard Larsen that the
waste line was a temporary hook up and that a new waste pipe
system was going to be installed when his part of the job was
complete.
As you know, I have been in contact with a Mr. John F.
Magas from Consumer Affairs. I will be forwarding a copy of
this letter to him along with an inspection report. Mr. Larsen
will also be receiving a copy of this letter.
I would also like to remind you at this time that it is
unlawful to occupy that part of the building under construction
covered by the Building Permit. A CO must be obtained before
that part of the building can be occupied.
In closing I hope for a speedy resolve to this matter. If
you have any questions or if I can help within my capacity as a
Building Inspector for the Town of Southold, please call be at
765-1802 anytime between the hours of 8:30-4:30 Monday to Friday.
