All dredging shall be conducted so as to leave a uniform bottom elevation free of mounds or holes. L. All spoil material shall be disposed of either in the Atlantic Ocean or at an approved U.S. Government Dumping Ground, as directed by the District Engineer, U.S. Department of the Army, Corps of Engineers. Any spoil material not intended for such water disposal shall be removed from the site and disposed of at an approved upland site. . X M. All fill and spoil material shall be suitably contained at the work site so as to prevent its eroding, leaching or otherwise entering into adjacent wetlands and waterways. _ N. All peripheral rip-rap berms, cofferdams, rock revetments, gabions, bulkheads, etc. shall be completed prior to placement of any fill material behind such structures. 0. All repairs to existing structures shall be confined to replacement of existing structural elements with no change in design, dimension, or materials, unless specifically authorized herein. _ P. If any of the permit conditions are unclear, the permittee shall contact the Division of Regulatory Affairs at the address and telephone noted below. I B. A copy of this permit or approval and supplementary conditions shall be available at the project site whenever authorized work is in progress. C. The permit sign enclosed with the permit or a copy of letter of approval shall be protected from the weather and posted in a conspiciuus location at the work site jintil completion of authorized work. D. At least 48 hours prior to commencement of the project, the permittee shall complete and retu~n the top portion of the enclosed receipt form certifying that he is fully aware of and understands all provisions and conditions of this permit. Within one week of completion of the permitted work, the bottom portion of', that form shall also be completed and returned. E. For projects involving activities to be accomplished over a period or more than one year, th,e permittee shall notify the Regional Permit Administrator in writing at least 48 hours to the commencement of resumption of work each year. I F. If project design modifications take place after permit issuance, the permittee shall submit the appropriate plan changes for approval by the Regional Permit Administrator prior to undertaking any such modifications. The permittee is advised that substantial modification may require sub- mission of a new'iapplication for permit. Instructions for Final Approval of Constructed Systems It is the applicant's responsibility to call the Department to arrange inspections of the septic system and water supply facilities prior to backfilling. This includes inspections of the soil excavation for the septic system and inspections of the water supply well, well lateral, public water supply line, septic system and piping. Following satisfactory construction and inspections: 1. The applicant must submit 4 prints of a survey (up to and including 11"x17"), by a licensed surveyor, of the subject property showing the following: a. The lot location and dimensions. b. The lot number(s) and the name of the subdivision, if applicable. c. The final elevations of the lot. d. The exact location of the private well, if applicable (.give at least 2 dimensions measured from the corners of the building). e. The exact location of the public water service line, if applicable. f. The exact location of the septic tank and leaching pool(,s), if applicable. Give 2 dimensions from the building corners to the centers of the septic tank and each leach- ing pool. g. The exact location of the sewer line from the dwelling to the street, if applicable. h. A clear area at least 2"x4" for the Department's approval stamp. 2. The applicant must submit a certificate from the sewage disposal system installer attesting that the system has been installed according to the criteria of the Suffolk County Department of Health, when applicable. 3. The applicant must submit a well water analysis and a well driller's certificate, when applicable. If the well or water quality does not conform to standards, proof of corrective measures will be required. 4. In those cases where public sewers are utilized for the dwelling, the applicant is also to submit one copy of the sewer line inspection approval from the public sewer district. In districts operated by Suffolk County, 2 copies of Form S-9, duly executed by the Suffolk County Department of Public Works, are required. 5. In those cases in which the installation and connection of the public water service line have not been inspected by the Department of Health, a tap letter from the appropriate water company is required. If the lot is part of anlold filed map (prior to Januray 1, 1960) or is a described parcel, and involves private sewage disposal, then the following procedure is to be followed: A. Submit 3 prints of al,plot survey (up to and including 11"x17") by a licensed surveyor containing the following information: 1. The applicant's name, address and telephone number clearly printed or typed. 2. The following statement signed by the applicant-"The water supply and sewage disposal systems for this residence will conform to the standards of the Suffolk County Department of Health." 3. Lot location, dimensions, map name and number, and lot number (if applicable). 4. Location of all urface waters within 300' of the applicant's lot. 5. Elevations of al corners and center-line street elevations at property corners. 6. Proposed dwellin and any existing dwellings within a 100' radius of the property (indicate if vaunt). Indicate the source and location of water supply and sewage disposal systems for these dwellings. 7. Distance and location of the nearest public water main. 8. Page, block and arcel number for the lot, as shown on the Suffolk County Tax Maps. 9. Proposed locatio~ of water and sewage disposal systems (will be honored if it meets standards)'. 10. If the lot is part of a 'subdivision map approved by the Suffolk County Department of Health after January 1, 1960''„ and involves private sewage disposal systems, submit 3 prints of a survey by a licensed surveyor containing Items 1 through 9, Paragraph A. 3. In areas where there is !less than 5' from existin§ grade to groundwater, a grading plan showing the sewage disposal system location and proposed grading is required. 4. If the lot is to be served by public water and sewers, submit 3 prints of a survey by a licensed surveyor containing; Items 1 through 8, Paragraph A. The surveyor should also include on the survey the location of the sewer stub in the street and a statement as to whether the sewers are operational. In Dieu of this, an approval from the Sewer District, showing the stub location and availabillity of sewers will be accepted. If a temporary sewage disposal is necessary, a test hole isl, required as described above (Item 10, Paragraph A). 5. If the lot is within 300' of any body of water, approval from the appropriate agency having jurisdiction must accompany 'Ithis application. 6. Applicability M. As required by nuffolk County Sanitary Code Article 4, Sections 406 through 409, all new~and replacement individual home water supply systems must conform to the !,standards contained in this document. An individual home water supply system is defined as a single private water system con- sisting of a well, Piping, tank(s), water treatment unit (if necessary), and other facilities 'intended to supply water to a single-family residence. The replacement of an individual home water supply system includes well relocation, change ofi screen depth, and/or screen replacement. 2. Well Yield Individual home I, Water supply system wells must provide a minimum yield gallons per ml of five inute (measured at the influent to the storage tank). 3. Well Depth and Location The top of the 'iwell screen must be installed at least 40 feet below the water table and liat least 50 feet below grade. Wells screened within perched water will not be approved. For wells installed near bodies of salt water, the top o'f the screen may be installed at a shallower depth (up to a minimum of ten feet below the water'table) if the applicant can prove to the Department's satisfaction that it is not possible to obtain water of satisfactory quality (land quantity at the required or greater depths. For such wells, the horizontal distance to the nearest leaching pool must be increased by one foot (beyond the normally required 100 feet) for each foot that the top of the well screen is less than the required 40 feet below the water table.
