APPLICATION FOR CERTIFICATE OF OCCUPANCY
A. This-application must be filled in by typewriter OR ink and submitted to the building
inspector with the following: for new building or new use:
1. Final survey of property with accurate location of all buildings, property lines,
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains
less than 2110 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings
and installations, a certificate of Code Compliance from architect or engineer
responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
3. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and
"pre-existing" land uses: -
1. Accurate survey of property showing all property lines, streets, building and
unusual natural or topographic features.
2. A properly completed application and a consent to inspect signed by the applicant.
If a Certificate of Occupancy is denied, the Building inspector shall state the
reasons therefor in writing to the applicant.

replacement - Status of
Dear Mr. Horton:
Investigation of column replacement work has determined that original
design of this Office was not followed by the Subcontractor,
Necessary additional welded steel reinforcing has been designed, as
shown by prints in your Office, submitted this date, to provide full
column strength for full loading conditions.
Under present minimum winter congregation conditions, the structure is
entirely safe for use at the present time. The upgrading will be done
as soon as release is effected.
It will be greatly appreciated if the Greek Orthodox High Holy Days
may be celebrated in the Church Edifice this Friday, Saturday and
Sunday.

Construction Specifications
1 - Contractor shall contact Church prior to submitting Bid for
Column Removal and iteolacement to ascertain Church's future
use of area involved.
2 - Contractor shall submit work schedule and method of work and
enclosure, support structures and other data required, to
Church for tLeir apn-roval.
3 - Contractor shall carry all necessary Insurance required holding
the Church and 'i~nKineer free from any harm.
4 - Contractor shall sub-nit shop draw--ngs on steel :c^rk to -n~7ireer
for his oversee.
5 - Contractor shall clean up the site of work daily and provide a
dumpster at rear of i4ifice for debris.
6 - Contractor shall contact Church and EnFzineer would any unforseen
problems develop when existing column areas are opened so that
immediate remedy can be effected.
7 - No field welding or flame cutting allowed in Edifice.

8 - No single column/pole support allowed, Contractor shall support
- floor above (Sanctuary) maintaining it's original level at all times.
9 - Contractor shall remove and replace ceiling tile, fixtures, as
required to return area to original condition.
10 - Contractor shall keep dust to minimum in the cutting of concrete
floor, pouring of new footing (boxed), replacement of floor and
new matching floor tile.
11 - Contractor shall submit payment schedule required, payments minus
10o shall be made as approved by Lngineer and submitted to Church
for payment.

12 - Final Payment to Contractor shall be made upon submission of
final bill, together with clearance statement stating that no
liens etc. are in effect and that all bills have been paid.
13 - Contractor shall submit suggested method of enclosing-new
steel columns to Church for Approval i.e. "white Oak plywood
(or equal) 48" wainscot high with shatterproof mirror above
to ceiliniz tiles.
