A. This application must be filled in by typewriter OR ink and submitted to the building
inspector with the following: for new building or new use:
1. Final survey of property with accurate location of all buildings, property lines,
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form).,
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings
and installations, a' certificate of C~de Compliance from architect or engineer
responsible for the building.
6. Subm%t Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and
"pre-existing" land uses:
i. Accurate survey of property showing all property lines, streets, building and
unusual natural or topographic features.
2. A properly completed application and a consent to inspect signed by the applicant.

Analysis of the concrete footings reveals that choy have been set
too high in relation to the ~rost line.
As per Uniform Bnllding Code Section 2907 '~sotings':
"Footings and foundations, unless otherwise specifically provided
shall be constructed oi masonry, concrete or treated wood in
conformance with UBC Standard No,29,3 and in ali cases shall
extend below the frost line."
The bottom of the footing currently s~ts at 28 1/2" below grade
The top of the footing should set at $6' below grade (frost line)
in order to meet code requirements,
When pouring the corrected footings, place a spacer between ~he
footing and the rat slab in order to allow ior mow~ment.
Reinforcing at the footing is recommended, Moisture Carrier under

This is to confirm our agreement for professional services as related to
the above property. I have agreed to prepare design and construction documents
for the proposed addition and to file them with the Building Department of
Southold for approval and required permits. We have agreed that my
professional fee for these services will be ~dollars ~
plus expenses. These expenses will include filing fees, reproduction costs,
and reimbursement for professional consultants if required.
If this concurs with your understanding of our agreement please sign
one copy of this letter and return it to me with a deposit of $250.00.
