APPLICATION FOR CGRTIPICATC OF OCCUPANCY
T}tis.application must be filled in by typewriter OP, ink and submitted to the building
inspector with the following: for new building or new use:
1. Final survey of property cvith accurate location of all buildings, property lines,
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board o£ Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of 17, lead.
5. Commercial building, industrial building, multiple residences and similar buildings
and installations,.a certificate of Code Compliance from architect or engineer
t responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and
"pre-existing" land uses;
1. .lc curate survey of property showing all property lines, streets, building and
unusual natural or topographic features.
2• A properly completed application and a consent to inspect signed by the applicant.
If a Certificate of Occupancy is denied, the Building Inspector shall state the
reasons therefor in writing to the applicant.

Dear Mr. Miller & Mr. Powell:
On February 21, 1992 you applied for a Building Permit for
a Solid Fuel Burning Appliance in the non-habitable accessary
building. You also left us your check #376 in the amount of
$100.00 for the fee far the permit and for the fees for the
Certificate of Occupancies.
A Certificate of Occupancy for Building Permit #20210-Z for
the alteration to the non-habitable accessary garage cannot be
issued because we still need the Underwriters Certificate for
electrical work that was done. Please send us a separate check
in the amount of $25.00 for the CO along with the Underwriters
Certificate.
We are returning herewith your check in the amount of
$100.00. Please send us a new check in the amaunt of $75.00 so
that we may issue the Building Permit and CO for the fuel
burning appliance. Also we will be able to issue the CO for
Building Permit #16504-Z for the brick patio. This building
permit has expired.
If you have any questions, please do hesitate to call this
office. Thank you for your cooperation in this matter.
