2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form ,

Dear Sirs/Madams,
Enclosed is the application for the Certificate of Occupancy. I have filed it out for
"additions to dwelling" and "New Construction", per instructions from your office,
and a check for $25 is enclosed to cover the fees.
The electrical Underwriters Certificate was dropped by at your office on Monday,
11/30, by the contractor, Charles Thompson. I understand that that form, and the
Plumber Solder Certificate are both now on file.
In case you need them for any reason, T enclose a survey of the property and the
existing C. of O., with an update from the Title Company, concerning the removal of
sheds and fences shown on the Survey. As no area was added to the footprint of the
building, as the addition replaced an existing one, I hope that this is sufficient.
If there are any questions or I need to give you more information, would you let me
know as soon as possible, as the Bank is holding escrows pending a C.of O. and I
need to use them to complete the contract with the contractor. I can be reached
