This application must be filled in by typewriter OR ink and submitted to the building
inspector with the following: for new building or new use:
1. Final survey of property with accurate location of all buildings, property lines,
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of i% lead.
5. Commercial building, industrial building, multiple residences and similar buildings
and installations, a certificate of Code Compliance from architect or engineer
responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and
"pre-existing" land uses:
l. Accurate survey of property showing all property lines, streets, building and
unusual natural or topographic features.
2. A properly completed application and a consent to inspect signed by the applicant.
If a Certificate of Occupancy is denied, the Building Inspector shall state the
reasons therefor in writing to the applicant.

check for $25 for alterations at 111-85 Soundview Ave.,
Southold. As per your letter to me of 6/29 I believe the
only remaining information you need is an underwriters
certificate. I have have completed the electrical inspect-
ion and expect to recieve the certificate shortly. I will
forward it to you immediately. In the meantime, please let
me know if there is anything else you will need.

Dear Sir:
Please be advised that on June 13, 1992 I personally visited the
above referenced premises and made the following observations with
regard to the New York State Energy Code:
Attic Floor Insulation is
9" Fiberglass batts ......................... = R-30
2. Exterior Walls are 2x4 studs
insulation is 3-1/2" Mineral Wool batts ...... = R-ii
Basement/crawlspace ceiling insulation
is 3-1/2" Fiberglass batts ................... = R-ii
4. The proportion of glass area to insulated exterior wall
area is 23.7%
The windows and patio door on the Northerly portion of the
premises are insulated glass as required. The remaining win-
dows and door will have to be fitted with storm panels so as
to be in compliance with the code.
6. The existing fireplace has a damper which appears to be in
good and serviceable condition.
A new oil-fired heating system was installed prior to January
1, 1987. As required it is controlled by a thermostat and
vented through a new masonry chimney.
It is my opinion that upon installation of the above referenced
storm windows and storm door, the building envelope of this pre-
mises will be in compliance with the New York State Energy Code
which was in effect on the date the heating system was installed.
