This application must be filled in by typewriter OR ink and submitted to the building
inspector with the following: for new building or new use:
1. Final survey of property with accurate location of all buildings, property lines,
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings
and installations, a certificate of Code Compliance from architect or engineer
responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and
'!pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and
unusual natural or topographic features.
2. A properly completed application and a consent to inspect signed by the applicant.
If a Certificate of Occupancy is denied, the Building Inspector shall state the
reasons therefor in writing to the applicant.

At your request, we reviewed the attached vestibule plans
for the Mattituck Library dated October 8, 1992 The Planning
Board will grant a waiver of site plan approval. 'It would be
appreciated if you would direct the Library's representative to
write a brief letter to the Planning Board for our records, to
which we'will respond immediately. For your convenience, a copy
of the waiver procedure is enclosed.

The purpose of this letter is to address an error in the
Planning Board's letter of site plan waiver, which was dated
October 23, 1992. In that letter, the entrance vestibule was
noted as being added to the east entry of the library. The
letter should have stated that the vestibule was being added to
the western entry to the library.
I hope this clears up any confusion that may have
resulted. Thank you for bringing this to my attention.

RE: PROPOSED ENTRANCE VESTIBULE FOR PUBLIC DOOR TO
DESCRIPTION:
Construct a new vestibule wittl colonial trim coordinated to the existing build-
ing. Tile roof and wails of this new vestibule wouId cover the entire existing
slab at tile west side public and employees entrance. The vestibule would have
built-up trimmed wood columns on either side of [ts aluminum/glass entrance door.
Aluminum/g]ass wails would return from these columns to the Library's brick ex-
terior walls. Tile existing Library's west entrance and employee doors would be
removed and repIaced with a new aluminum/glass and hollow metal doors respectiv-
ely. Panic bar and cioser hardware would be provided on both new public doors.
The new employee door is to have a lockset and closer. Vestibule ceiling will be
3/8" wood T&G yellow pine bead board with 2 recessed Iight fixtures. One flood-
light will be mounted on exterior wood fascia and be controlled by existing
photo cell on rerouted circuit.
The roof of the vestibule will be a slightly pitched, flat rubber roof with new
glitters carrying tile run off water to existing storm drainage pools. The roof
edge trim orl the vestibule will match the existing trim on the west wail of the
Library south of the new vestibule, interior wa]l's brick will be left as is.
New interior walls shail be trimmed with pine and painted white above and beside
the aluminum and glass components. Tile existing air conditioner freon lines
sha]l be enclosed with wood Erim inside the new vestibule. Ail wood trim shal!
be primed on all surfaces prior to installation. The wood trim and hollow met~door
door shall be painted with oi1 based primer and exterior tFim paint. Existing
slab in vestibule area will be covered with "Rubber Squares" "Duratile" standard
12" x 3/8" tllick by Pawling Corp.
If required, additional foundation work will be done to support the new
roof's load at tile built up co]umns beside the new exterior door.
