Note that a Wetlands permit for the dock structure was not found in town records.
David Bergen on behalf of NANCY COLLINS requests a Wetland Permit to replace 144' of existing
bulkhead including a ±17.4' long south return in-place using vinyl sheathing, 10" diameter pilings 6'
on center, TxT whalers, 16' tie-rods leading to vertical deadmen with horizontal lay logs, and a
non-treated timber cap; temporarily remove and replace one (1) existing 4'x8' cantilevered platform
to existing north 2'2"x10' aluminum ramp with handrails to existing 6'x18' floating dock, and existing
south 2'3"x10' aluminum ramp with handrails to existing 6'x18' and 6'x20' floating docks; replace in-
place 4' wide by±126' long detached non-treated wood walkway with 4"x4" posts (walkway
approximately 6" landward of bulkhead); install water and electrical connections; remove existing
southerly retaining wall and contour land to match adjacent; reclamation dredging to a maximum of
4' depth at mean low water along entire length of bulkhead, to extend to a maximum of 10' off
bulkhead resulting in ±10 cubic yards of material to be placed behind bulkhead; and to install and
perpetually maintain a 10' wide non-turf buffer (including 4' wide non-treated wood walkway) along
the landward edge of the bulkhead. Located: 305 Dawn Drive, Greenport. SCTM# 1000-35-5-20
The proposed action has been reviewed to Chapter 268, Waterfront Consistency Review of the
Town of Southold Town Code and the Local Waterfront Revitalization Program (LWRP) Policy
Standards. Based upon the information provided on the LWRP Consistency Assessment Form
submitted to this department and the records available to me, I recommend that the proposal is
CONSISTENT with LWRP Policies and with the LWRP.
The replacement of one (1) existing 4'x8' cantilevered platform to existing north 2'2"x10' aluminum
ramp with handrails to existing 6'x18' floating dock, and existing south 2'3"x10' aluminum ramp with
handrails to existing 6'x18' and 6'x20' floating docks is recommended as INCONSISTENT.
Pursuant to Chapter 268, the Board of Trustees shall consider this recommendation in preparing its
written determination regarding the consistency of the proposed action.

A. INSTRUCTIONS
1. All applicants for permits* including Town of Southold agencies, shall complete this CCAF for
proposed actions that are subject to the Town of Southold Waterfront Consistency Review Law. This
assessment is intended to supplement other information used by a Town of Southold agency in
making a determination of consistency. *Except minor exempt actions including Building Permits
and other ministerial permits,not located within the Coastal Erosion Hazard Area.
2. Before answering the questions in Section C, the preparer of this form.should review the exempt
minor action list, policies and explanations of each policy contained in the Town of Southold Local
Waterfront Revitalization Program. A proposed action will be evaluated. as to its significant
beneficial and adverse effects upon the coastal area(which includes all of Southold Town

affect the achievement of the LWRP policy standards and conditions contained in the consistency
review law. Thus, each answer must be explained in detail, listing both supporting and non-

Replace existing failing bulkhead and return in place with a total of approximately144 ft of vinyl
sheathing, 10 inch pilings 6ft on center, 3 6X6 whalers, 16 ft. tie rods leading to vertical
deadman with horizontal lay logs,finish with non-treated lumber cap; one 6'X8' cantilevered
platform, a north aluminum ramp 10 ft. X 2 ft 2 inches long with handrails, a south aluminum
ramp 10 ft. 6 inches X 2,ft 3 inches with handrails, water and electrical connections and replace
yin place 4 ft wide by approximately 126 ft. continuous detached non-treated walkway with
