APPLICATION FOR CERTIFICATE OF OCCUPANCY ~
A. This application must be filled in by typewriter OR ink and submitted to the building
inspector with the following: for new building or new use:
1. Final survey of property with accurate location of ail buildings, property lines,
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4, Sworn statement from plumber certifying that the solder used in system contains
· less than 2/10 of 1% lead.
5. Conmtercial building,' industrial building, multiple residences and similar buildings
and installations, a certificate of Code Compliance from architect or engineer
~ responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and
'.'pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and
unusual natural or topographic features.
2. A properly completed applicJtion and a consent to inspect signed by the applicant.
If a Certificate of Occupancy is denied, the Building Inspector shall state the
reasons therefor in writing to the applicant.

ZBA Actions of April 2, 1992
In order to allow the applicants under the following ZBA
applications to reactivate their building permit applications,
please let this confirm the following actions and conditions
taken by the Board at our April 2, 1992 meeting:
I. Appl. No. 4089 - EVELYN P. TURCHIANO. Approval granted for
20 ft. x 20 ft. accessory garage structure in the front yard
SUBJECT TO:
(a) a setback at not less than 20 feet from the front
property line;
(b) a setback at not less than 5 ft. to the southerly
side property line;
(c) a height not greater than 16 ft., as applied;
(d) only utility shall be electric {no plumbing
facilities proposed in this application};
(e) garage door shall not face the street, as applied
and as shown on the plan revised 3-21-85 submitted under this
application.
II. Appl. No. 4096 - PAT AND ROSEANNE IAVARONE. Approval
granted for a proposed open deck addition to existing dwelling
SUBJECT TO:
(a) the deck remaining open and unroofed, as applied;
(b) the setback shall be not less than 54 feet from the
top of the bluff, as applied;

c. The work covered by this application may not be cbmmenced before issuance of Building Permit.

e. No building shall be occupied or used in whole or in part for any purpose whatever until a Certificate of Occupancy
shall have been granted by the Building Inspector.
