APPLICATION FOR CERTIFICATE OF OCCUPANCY
This application must be filled in by typewriter OR ink and submitted to the building
inspector with the following: for x~ew bulldiug or new nse:
1. Final survey of property with accurate location of all buildings, property lines,
streets, and unusual natural or topographic features.
Final Approval from tiealth Dept. of water supply and sewerage-disposal(S-? form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from [>lumber certifying that the solder used in system contains
±ess than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings
and installations, a certificate of Code Compliance from architect or engineer
responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and
"pre-existing" land uses:
1. Accurate survey of property showing all property l~nes, streets, building and
uuusual natural or topographic features.
2. A properly completed application and a consent to inspect signed by the applicant.
If a Certificate of Occupancy is denied, the Building inspector shall state the
reasons therefor in writing to the applicant.

Dear Building Official:
Pursuant to your department's request and being the Architect
I have visited and investigated the on site conditions of the
under construction at the above referneced location.

I have examine the cantilever condition of the joists over the top of
the foundation wall between the garage and the master bathroom area.
The cantilever of the joists is only about eighteen (18) inches. The
joists are long enough and fastened well enough, in my opinion, to posed
no threat to the structure of the dwelling. The required fire rating
separation of the garage from the dwelling should however, continue down
the wall and along the underside of the joists back to the concrete
wall, to maintain the fire rated construction.
I have also looked at the area of the proposed "haunch slab" condition
under the existing post near the foyer/entry area. The contractor
proposes to pour a "haunch slab" approximately 12" thich and 18" square
under the corner posts at the intersection of the family room wall and
the foyer. I feel that this type of underpinning of that area will be
sufficient to support the corner posts and posed no threat to the
structure of the dwelling.
I therefore believe that construction should be allowed to continue as
proposed. If you should have any questions about the foregoing, or any
aspect of the project, please do not hesitate to contact me.
