streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form),

4. Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of 1% lead.

and installations, a certificRte of Code Compliance from architect or engineer
responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.

I own the property at 960 Pequash Avenue, Cutchogue, map no. 473889-103-11. As per
a conversation with Jane in your office, please accept this letter as a'formal complaint of
the property on the north side of mine. This property belongs to the late Bud McCaffery
(Champ) who kept it manicured inside end out. The property is now being maintained
by Mr. McCaffery's son, Berni.f), who appears to be renting the home. The property is
no longer maintained angers to have, in my opinion, some building violations. Right
now there are a couple of boats, a utility trailer and several sheds on the property. One
of the sheds, approximately 8' x 15' seems to be sitting on the property line if not on my
property. There is a 20' travel trailer within three feet on the side of my property. In a
conversation with Mr. Fisher from your office, any shed shouid be 15' to 20' from the
property line. I was told the travel trailer should be in the rear of the property with
adequate distance from the rear property line.
I have on numerous occasions contacted Mr. McCaffery's son to alert him of the
situation. I have also attempted to express my concerns to the tenants but to no avail.
I would appreciate someone looking into this matter for me. I can be reached at 589-
5399 should you need any additional information. I would also like to be informed of the
outcome of your investigation. My primary residence is 10 Greentree Drive, Oakdale, NY
11769.
