Carrell O Southold, New York 11971 Peter Wenzel Telephone (516) 765-1892 Fax (516) 765-1823 BOARD OF TOWN TRUSTEES %TOWN OF SOUTHOLD August 25, 1994 V s' Mr. Theodore Petikas 225 Stewart Ave Bethpage, NY 11714 Re: SCTM #135-1-27 Dear Mr. Petikas, The following action was taken by the Board. of Town Trustees during its regular meeting held on Aug, 1994 regarding the above matter: WHEREAS, THEODORE PETIKAS applied to the Southold Town Trustees for a permit under the provisions of the Wetland Ordinance of the Town of Southold, application dated June 13, 1994 WHEREAS, said application was referred to the Southold Town Conservation Advisory Council for their findings and recommendations, and WHEREAS, a Public Hearing was held by the Town Trustees with respect to said application on August 25, 1994 at which time all interested persons were given an opportunity to be heard, and, WHEREAS, the Board members have personally viewed and are familiar with the premises in question and the surrounding area, and, WHEREAS, the Board has considered all the testimony and documentation submitted concerning this application, and, WHEREAS, the Board has determined that the project as proposed will not affect the health, safety and general welfare of the people of the town, NOW THEREFORE BE IT, RESOLVED, that the Board of Trustees approves the application of THEODORE PETIKAS to place 52' of rock wall to prevent erosion, construct a 14` X 20' deck and a 41 high fence on west side of property and a 6' high fence on east side of property. Located 52755 Soundview Ave., Southold. Permit to construct project will expire two years from the date it is signed. Fees must be paid, if applicable, and permit issued within six months of the date of this notification. County Road 38 Southold, N.Y. Dear Mr. Petikas, The pile were 20' in length, 10" butt, pressure treated with CCA to 1 lbs./cu.ft. The piles were driven with a Delmag diesel pile crane developing 9100 ft. lbs. of energy with fixed lead to a minimum of 30 tons per pile. The proposal is for a gradual slope away from the septic system on its east side, eliminating the need for a retaining wall. This regrading would involve a minimal amount of fill on the Town property in order to maintain a 5% slope within 20 feet of the cesspools. Attached is a copy of the revised plan. Give 2 dimensions from the building corners to the covers of the septic tank and each leaching pool; g. the exact location of the sewer line from the dwelling to the street; if applicable; and h. have a clear area at least 3"n~5" for the Department's approval stamp. 2. The applicant must submit a cert i ificate from the sewage disposal installer attesting that the system has been installed according to the criteria of the Suffolk County Department of Health Services, when applicable. 3. If a well has been installed as the potable water supply, the applicant must submit a current well water analysis (within one year) and a well driller's certificate. If the well or water quality does not conform to standards, proof of corrective measures will be riequired. Refer to "Standards and Procedures for Private Water: Systems, 4. In those cases where public sewers are utilized for the dwelling, the applicant is also to submit one t l) copy of the sewer line inspection approval from the public sewer district.
