APPLICATION FOR CERTIFICATE ~F OCCUPANCY
This application must be filled in by typewriter OR ink and submitted to the building
inspector with the following: for new building or new use:
1. Final survey of .property with accurate location of all buildings, property lines,
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains
less than 2/I0 of i% lead.
5. Commercial building, industrial building, multiple residences and similar buildings
and installations, a certificate of Code Compliance from architect or engineer
responsible for the building.
6. Submit Planning Board Approval of c6mpleted si~e plan requirements.
For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and
"pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and
unusual natural or topographic features.
2. A properly completed application and a consent to inspect signed by the applicant.
If a Certificate of Occupancy is denied, the Building Inspector shall state the
reasons therefor in writing to the applicant.

We would like to apply for an extension of the above building permit. Due to the fact that we
have not complete the upstairs of our house and we it will not be completed before the above
permit expires.

Conversiog of secon~ floor area into usable living space &
construction of dec~ on south east corner of house as
follows:
(1)
Insulation:
Ceiling; 10" R-30 kraft faced fiberglass.
Exterior walls; 6 1/4" R-19 kraft faced fiberglass.
Exterior Walls, (closet areas); 4" kraft faced
fiberglass. To provide for ventalation from first
floor soffits to second floor attic space where
required.

Framing:
(A)
Existing closet space on north & south east corners
will be enlarged from 54 1/2" x 34" to 54 1/2" x 66
1/2" using 2x4 & 2x6" lumber 16" o.c. sliding doors
will be installed at a latter date.
(B) Existing closet oppening on north wall ( center )
will be framed to accept 36" door from stairwell
oppening as shown on plans.
(C) New bath will be constructed as shown on plansusing
2 x 4" lumber 16" o.c. with single bottom & double
top plates.
(D) Window seating: Platforms will be constructed using
2 x 4" lumber 16" o.c. doubled accros front,
covered with 1/2" AC plywood & installed approx.
29" above floor.

Existing stairwell wall (west) will be reduced in
height to approx 48". Floor to ceiling cotoumn
will be installed at south end of wall & ceiling
above w!!~ be framed as requiredto provide anchor
point for top of coloum~.
Existing stairwell wall (east) will be extended &
framed to accept 36" door as shown on plans.
(G) Existing closet space on north & south west corners
will have sliding doors installed at a latter date.
(H) Sheetrock: 1/2" sheetrock will be used on ceiling &
all walls except for bathroom where 1/2" water
resistant sheetrock.
Flooring: 1/2" plywood will be installed over
existing subfloor. Carpet, lino, tile or wood
flooring will be installed at a latter date.
Exterior deck to be constructed as follows:
Ail lumber will be CCA.
All framing will be 16" o.c.
Deck frame will be bolted & nailed to house using
~ 20 nails & 3/8" x 4" lag bolts with washers.
Flashing will be installed between existingstructure
& new construction.
All Framing against house will have teco brackets
installed as required to provide adequate support.
Girders: To be constructed from 2 x 6" CCA & installed
as shown on plans.
Floor joists: To be constructed from 2 x 8" CCA as shown
on plans.
Decking: 2 x 6" CCA spaced approx 1/8" for drainage.
Railings: Will be supported by 4 x 4" posts & will be
constructed from 2 x 4" & 2 x 2 "CCA lumber.
Girder support posts will be 4 x 4" CCA supported by an
10" dia. 8" thick footing~ 36" below surface of
ground. /
