This application must be filled in by typewriter OR ink and submitted to the building inspector with the following: for new building or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final Approval from Health Dept, of water supply and sewerage-disposal(S-9 form). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 19 lead. 5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate of Code Compliance from architect or engineer responsible for the building. 6. Submit Planning Board Approval of completed site plan requirements. B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and pre-existing" land uses: 1. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic features. 2.. Suff. Co. Tax Map #1000-78-9-68 To Whom It May Concern: After conducting a framing inspection at the above premises on June 16, 1993 I found the ceiling height to be less than the required 716" (reference N.Y.C.R.R. Part 711.1 (a) (2). I brought this fact to the attention of the contractor, Mr. Robert Bohn. He assured me at that time he would correct the problem. I went back to the premises on June 22, 1993 and found the height problem was in fact corrected and issued him an OK on the framing. A Certificate of Occupancy was issued on June 30, 1993 and should assure all concerned that this renovation/alteration meets the requirements of the N.Y. State Building Code.
