Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form)· 3. Approval of electrical installation from Board of Fire Underwriters. 4. This certificate was issued to Donald Bracken and Diane Carroll. A Building Permit #21447-Z was issued 6/2/93 to the present owners Jeffrey S. & Donna M. Biggs for a deck addition to the above mentioned dwelling. This permit was amended 8/9/94 to include alterations to finish off the second floor.
