A. This application must be filled in by typewriter OR ink and submitted to the building
inspector with the following: for new building or new use:
* 1. Final survey of property with accurate location of all buildings, property lihes,
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form).
· 3, Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings
and installations, a certificate of Code Compliance from architect or engineer
responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B.. For existing buildings (prior to April 9 1957) non-conforming uses, or buildings and
-pre-exzsting" land uses:
1.Accurate survey of property showing all property lines, streets, building and
unusual natural or topographic features.
2. A properly completed application and a consent to inspect signed by the applicant.
If a Certificate of Occupancy is denied, the Building Inspector shall state the
reasons therefor in writing to the applicant.

Dear Mr. Fish,
Attached please find a letter dated July 21, 1994 from Mr. Michael Gatoff
to Schneider/Knapp in regard to certain underground drain piping at the
above referenced project.
It is our understanding that a stop work order was issued by your dep-
artment earlier this week when it was found that some underfloor piping
had been backfilled and slabed over prior to your inspection and approval.
Please be assured that this action was inadvertant, and due to the contrac-
tors obvious concern to keep the store open for business and in a con-
dition safe for customers and employees, and not any attempt to conceal
sub-standard work.
The reputation and experience of Gatoff Mechanical Group, Inc., the
plumbing subconstactor, in the type of construction involved here gives
the Architects full confidence that the piping put in place and covered
prior to your inspection was, in every aspect, installed in accordance with
the plans, specifications and all applicable codes.
It is hoped that these statements along with the exposure of a portion of
the buried piping which you requested will suffice to have the stop work
order lifted after your inspection on Monday July 25.

Dear Mr. Knapp,
I am writing ~his letter to confirm our conversation regarding the' above renovation project and
the current problem related to it.
My company is a professionally managed, thirty one (31) year old, .commercial contracting firm
which sceciMizes in the supermarket industry throughout New York and New Jersey.
We are licensed in all territories in which we operate and possess an unblemishe~_t_ record in all
municipalities where we have suceessful/y completed over one ~thousand (1,000) projects to date.
In shorh we are a totally legitimate Plumbing and Fire Sprinkler Contractor wbo~ reputation
has been earned over many years of effort and we are proud of it. '
Our employees are members of all the Unions in each respective territory in which we work,
including this particular project I am presently writing to you about.
A situation has developed where a "stop work order" has been issued for the project.
Our employees have installed a portion of the new/revised underground drainage system at this
location and inadvertently, the trenches were backfilled and cemented he. fore tl~e plumbing was
physically inspected by Mr. Gary Fish, the Inspector for $outhold.
At this time I would like to state emphatically that our underground cast iron drain lines which
were installed here were done so with all approved materials and in accordance with ail
applicable Plumbing Codes,
They have a/so been installed to the highest ~tandards. of our industry and I guaranty that they
are free of any defects tlmt would be associated with an inferior installation or use of sub-
standard materials.
I am the licensed Master Piumber for this project, have b~n associated with my firm for thirty
one (3 I) years and categorically state that thi~ installation has been performed correctly.
