APPLICATION FOR CERTIFICATE OF OCCUPANCY
A. This application must be filled in by typewriter OR ink and submitted to the building
inspector with tbe following: for new buildiog or new use:
i. Final survey of property with accurate location of ail buildings, property lines,
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of wmtet supply and sewerage-disposal(S-? form).
3. Approval of electrical installation from Board of Fire Underwriters.
d. Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings
and installa'tions, a certificate-of Code Compliance from architect or engineer.

PROVISIONS APPEALED FROM: Article IIIA, Section 100-30A.3.
RELIEF REQUESTED: Four (4') extension to living room which is 20
feet wide along front of dwelling, leaving a reduced setback from 37
feet to 33 feet (confirmed by owner in the application).
MOTION MADE BY: Goehringer SECONDED BY: Dinizio.
ACTION/RESOLUTION ADOPTED: Approved 4' x 20' wide addition, as
applied, in order to increase living room area, leaving a reduced front
yard setback at the requested 33 (or 32.9) feet. No reduction in side
yards to less than code requirements requested.
REASONS/FINDINGS: Variance requested is minimal at two feet from the
code requirement. No other alternative is available since the dwelling
is located as shown since prior to the enactment of zoning (prior to
1957). Property has frontage along two access routes and is
substandard in size (assessors' records show the property to be .30 of
an acre, or 16,000 sr. House has existed for over 50 years.
Substandard size and unusual dimensions of the land lend to difficulties
in conforming with the setback requirement.
