To protect the values of the tidal wetland, a permanent vegetated buffer zone shall be established. There shall be no disturbance to the natural vegetation or topography within an area extending 5o linear feet landward of the tidal wetland boundary. 7. A row of staked hay bales or approvable erosion control devices shall be placed at the landward edge of the buffer area or 50 linear feet from the property line, or as per NYSDEC approved plan at commencement of regulated activities and remain in place until project is completed and all disturbed areas stabilized with vegetation. 8. Roof runoff shall be directed to drywalls a minimum of 100 feet landward of the tidal wetland boundary for immediate on-site recharge. 9. Dry wells for pool filter backwash shall be located a minimum of 100 feet landward of the tidal wetland boundary. 10. There shall be no draining of swimming pool water directly or indirectly into tidal wetlands or protected buffer areas. 11. Roads, driveways, and/or parking areas shall be graded to direct runoff away from tidal wetlands and protected buffer areas. 12. The permittee shall incorporate the following language as a notice covenant to the deed: "Regulated tidal wetlands associated with Arshamomague Pond are located at Lot 2, "Long Pond Estates Section 1"Long Creek Drive, County Tax Map Number: District 1000, Section 052, Block 08, Lot 002, otherwise known as the properties of Lisa A. McElroy and her heirs, assigns or successors. This property is subject to the provisions of New York State Environmental Conservation Law (ECL) Article 25 or its successor, and the conduct of regulated activities may occur only pursuant to ECL Article 25 if prior approval is given by the New York State Department of Environmental Conservation (NYSDEC) or its successor. Regulated activities include, but are not limited to the erection of any structure(s); excavation; dredging; grading and filling; clearing of vegetation; and application of chemicals." These include inspections of the soil excavation for the sewage disposal system and inspections of the water supply well, well lateral, public water supply line, disposal system, piping and final grading. Other inspections may be required. Following satisfactory construction and inspections: L The applicant must submit 4 prints of an as-built plan (up to and including 11" ~ T ),[I y(IS li eri> e4]design professional, of the subject property showing the following: ' JUN 1 6 1905 a. the lot location and dimensions; _ i. - b. the lot number(s) and the name of the subdivision, if applicable; ;i c. permanent structures (i.e., buildings, driveways, walkways, swimming pools; flecks, etc.), - " d. the exact location of the private well, if applicable (give at least 2 dimensions measured from the comers of the building); e. the exact location of the public water line, if applicable; f. the exact location of the septic tank and leaching pool(s), if applicable. Give 2 dimensions from the building corners to the covers of the septic tank and each leaching pool; g. the exact location of the sewer line from the dwelling to the street; if applicable; and h. have a clear area at least 3"x5" for the Department's approval stamp. 2. The applicant must submit a certificate from the sewage disposal installer attesting that the system has been installed according to the criteria of the Suffolk County Department of Health Services, when applicable. 3. If a well has been installed as the potable water supply, the applicant trust submit a current well water analysis (within one year) and a well driller's certificate. If the well or water quality does not confonn to standards, proof of corrective measures will ;be required. Refer to "Standards and Procedures for Private Water Systems." 4. In those cases where public sewers are utilized for the dwelling, the applicant is also to submit one (1) copy of the sewer line inspection approval from the public sewer district. In districts operated by Suffolk County, two (2) copies of Form S-9, duly executed by the Suffolk County Department of Public Works, are required. 5. In those cases in which the,i nstallation and connection of the public water service line has not been inspected by the Department of Health Services, a tap letter from the appropriate water company is required.
