APPLICATION FOR CERTIFICATE OF OCCUPANCY
This application must be filled in by typewriter OR ink and submitted to the building
inspector with the following: for new building or new use: /
1. Final survey of property with accurate location of all buildings, property lines,
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings
and installations, a certificate of Code Compliance from architect or engineer
responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and
"pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and
unusual natural or topographic features.
2. A properly completed application and a consent to inspect signed by the applicant.
If a Certificate of Occupancy is denied, the Building Inspector shall state the
reasons therefor in writing to the applicant.

To the Building Inspector:
We've been wanting to meet with you to present the home addition
plans that we discussed, but unfortunately the weather has not
been cooperative.
You mentioned to me that it normally takes about ten days to
approve plans once they reach you, and since we're scheduled
to start April 1, we thought we could gain some time by mailing
the plans to you now and then meeting with you during your next
Fishers Island visit to answer any questions you might have.
Our builder is in the process of being licensed in New York.
Right now, we are undecided as to which New York licensed plun~er
we will hire but will give you his name at our next meeting.
Since we are doing the electrical ourselves, that should take
care of our licensing needs.
Please let us know when you expect to be at Fishers next. You've
been a great help giving us the needed information to date,
and we look forward to resolving any outstanding issues quickly.

Dear Tom:
We have discussed with our builder your four concerns and believe
we can provide the answers.
1. The microlam beams are 5 1/4" wide x 9 1/2" deep by 22'
long.
2. The attic insulation will be R-38.
3. The certification for the engineered trusses will be faxed
to you on Monday, March 26th by Matt Boggio, our contractor.
4. The R rating on the interior insulation will be raised to
R-13. Since the R rating on the rigid outside insulation is
5.3, that will total R-]8.5, me~ting New York regulations.
As you said, the 2x4 studding is OK as long as the total R rating
meets NY regs at R-18.
Hope this helps, Tom. We look forward to seeing you on your
next visit to the island.
