APPLICATION FOR CERTIFICATE OF OCCUPANCY
A. This application must be filled in by typewriter OR ink and submitted to the building
inspector with the following: for new building or new use:
1. Final survey of property with accurate location of all buildings, property lines,
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings
and installations, a certificate of Code Compliance from architect or engineer
responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and
"pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and
unusual natural or topographic features.
2. A properly completed application and a consent to inspect signed by the applicant.
If a Certificate of Occupancy is denied, the Building Inspector shall state the
reasons therefor in writing to the applicant.

Dear Mr. Fish:
On June 10, 1995, at the request of Mr. John Kyriakides, I performed an on-site
inspection at the above-captioned site to determine remedial measures to be taken to
remedy variances in construction noted by your department. Specific recommendations
are as follows:
(1) Sill plate elevation 3" above grade in lieu of spec 8" dowel #5 rebars with
non-shrink grout with 6" of embedment into the existing curb at 24" on centers. Form
and pour an additional 8" of concrete atop the original with new anchor bolts set in
place.
(2) Footing depth below grade 30" in lieu of spec 36" with new curb in place,
mound adjacent earth to 1" below the construction joint. This will create an overall
depth of 33" below grade. The material observed adjacent to and below footing is a
coarse free draining sand which is not subject to the formation of ice lenses and
therefore the chance of heaving is very remote.

(3) Width of wall footing 12" in lieu of 24" existing north and east footings
exceed the area needed for allowable bearing capacity. The west entrance footing will
require the addition of a 12" wide wall footing doweled into the existing at each side of
roll up door with a total length of 4 ft. from each comer point and a depth of 36". Care
is to be taken not to undermine the existing footing. Dowels are to be #5 rebar at 8" on
center in two rows: one at -1.0 ft. elev. and the other at -2.0 ft. elev. Dowels are
to be embedded 6" into existing concrete with a non-shrink grout.
