N.V. 11901.3397 - - 852-2100 18-380,12/92 - it INSTRUCTIONS gOR FINAL APPROVAL OF CONSTRUCTED SYSTEMS It is the applicant's responsibility to Fall the Department to arrange inspections of the sewage disposal system and water supply facilities prior to backfilling. These include inspections of the soil excavation for the sewage disposal system and inspections of the water supply well, well lateral,public water supply line, disposal system, piping and filial grading. Other inspections may be rgquired. Following satisfactory construction Illand inspections: 1. The applicant must submit 4 prints of an as-built plan (up to and including 11"x17"), by a licensed design professional, of the subject property showing the following: a. the lot location and dimensiot1s; b. the lot number(s) and the name of the subdivision, if applicable; c. permanent structures (i.e., buildings, driveways, walkways, swimming pools, decks, etc.); d. the exact location of the privy a well, if applicable (give at least 2 dimensions measured from the comers of the building); e. the exact location of the public water line, if applicable; f. the exact location of the septic tank and leaching pool(s), if applicable. Give 2 dimensions from the building corners to the covers of the septic tank and each leaching pool; g. the exact location of the sewer line from the dwelling to the street; if applicable; and h, have a clear area at least 3" 5" for the Department's approval stamp. 2. The applicant must submit a ceryl ificate from the sewage disposal installer attesting that the system has been installed according to the criteriIt of the Suffolk County Department of Health Services,when applicable. 3. If a well has been installed as the potable water supply, the applicant must submit a current well water analysis (within one year) and a well dril en's certificate. If the well or water quality does not conform to standards,proof of corrective treasures will be required. Refer to "Standards and Procedures for Private Water Systems." 4. In those cases where public sewrs are utilized for the dwelling, the applicant is also to submit one (1) copy of the sewer line inspection approval ftom the public sewer district. In districts operated by Suffolk County, two (2) copies of Form S-9, duly executll d by the Suffolk County Department of Public Works, are required. S. In those cases in which the itnstallation and connection of the public water service line has not been inspected by the Department of Health Services, a tap letter from the appropriate water company is required. HEALTH EPARTMENT REFERENCE NUMBER MUST BE ON ALL CO RESPONDENCE OR DOCUMENTS SUBMITTED. SUBMIT ALL N'ECESSARX FINAL PAPERS AT THE SAME TIME. PHOTOCOPIES OF DOCUMENTS WILL NOT BE ACCEPTED. Work involved in this contract is to be located at the Cephas Residence, west corner of King and Douglass Streets, Orient, N.Y. 2. The Owners referred to in these specifications is Mr. and Mrs. D. Cephas. 3. Contractor(s) shall furnish and install all material and equipment shown, listed, or described on the drawings or in these specifications, subject to qualifications, conditions, or exceptions as noted. Contractor shall furnish all labor, scaffolding, and tools necessary to complete the work. 4. Before ordering material or commencing work which is dependent for proper size and installation upon coordination with building or site conditions, contractor shall verify all dimensions by taking measurements at building site, and shall be responsible for their correctness. Any discrepancies between drawings and/or specifications and existing conditions shall be referred to the Architect for adjustment before any work affected thereby is begun. 5. Contractor shall take precautions necessary to protect adjoining properties from debris and excessive drainage and shall assume full responsibility for damages resulting from negligence in these items. No trespassing will be allowed on adjoining properties. 6. Contractor shall be responsible for providing and maintaining the following temporary provisions: a: Temporary water supply, including lines, valves, hose, and similar equipment for construction purposes. b: Temporary light and power, including wiring, lamps, and connections as required for the completion of the work. c: Temporary heat, as needed during cold and wet weather for the purpose of protecting and drying construction and materials. 10. All subcontractors shall be approved by the Owner prior to start of work. 11. The contractor shall, at all times, provide protection against weather, rain, wind, storms, frost, or heat, so as to maintain all work, materials, apparatus, and fixtures free from injury or damage. At the end of the day's work, all new work likely to be damaged shall be covered. If low temperatures make it impossible to continue work, the contractor shall cease work and shall so notify the Owner. Any work damaged by failure to provide protection as required herein shall be removed and replaced with new work at the contractor's expense. 12. Drawings are not to be scaled. 13. Contractors shall do all patching required due to removal of existing work and/or the installation of new work. 14. All new work shall match and meet flush to existing work as closely as possible unless otherwise noted. DIVISION 2 : SITEWORK GENERAL: Provide all labor, materials, transportation, equipment and services necessary to complete all sitework required by the drawings as specified herein, or reasbnable implied as necessary to complete the work. DEMOLITION 1. Interior areas exposed to the outside during demolition and construction to be protected from the weather at all times. 2. Contractor shall protect existing services to remain and shall notify all utilities and telephone services and make arrangements for hook-up, removal, or capping of equipment as necessary. Chop and repair existing foundation as required to make connection between new and existing basements as indicated on plans and to provide connection to new plumbing, heating and electric lines and drains. 4. Strip asphalt roofing, clapboard siding and building paper from existing house and remove all original window sashes (frames to remain). 5. Existing front porch, rear stoop, and areas of existing structure, as indicated on plans, to be demolished and debris removed from site. EARTHWORKS: Scope: Earthwork related to the general site, excluding earthwork within the building perimeter. 1. Strip and stockpile topsoil to south and east of proposed construction. Exact locations to be determined on site. 2. Excavate, trench, fill, backfill, etc. as required for the proper completion of the entire project, including furnishing additional fin as required. 3. Provide full subsurface septic system as indicated on plans approved by Suffolk County Department of Health Services. 4. Backfill to rough and finish grades as indicated on drawings. DIVISION 3 • CONCRETE GENERAL: Provide all labor, materials, transportation, equipment and services necessary to complete all concrete work required by the drawings as specified herein,F or reasonable implied as necessary to complete the work. CONCRETE Scope: This includes, but is not limited to: A. Formwork B. Accessories C. Cast-in-place-normal weight concrete D. All concrete shall be Three Thousand (3,000) p.s.i. after twenty-eight (28) days and shall conform to ASTM C94.78A "Ready-mixed Concrete". 2. Cement shall be Type 1-A conforming to ASTM C-150, with admixtures conforming to ASTM C-260 and C-226. 3. Aggregates shall conform to ASTXI C-33-78 "Concrete Aggregates. 4. Concrete proportioning shall be such that the quality and consistency shall be suitable for proper placing and required finishes. Slumps shall be as follows: FOOTINGS: 3" to 6" SLABS 3" to 6" 5. Reinforcing bars shall conform to ASTM A615. 6. Where spliced, reinforcing bars shall overlap minimum 6" and be securely wired together. 7. Wire mesh shall conform to ASTM A82 and ASTM A185. 8. Porous fill under slabs on grade shall be sound, durable crushed stone or gravel, well graded between 3/4" and No. 4 sieve size. 9. Forms shall conform to the lines, dimensions, and shapes of concrete indicated on the drawings. They shall be tight to prevent leakage of concrete. They shall be securely braced and maintained in position to prevent any possibility of movement after the concrete is placed and shall insure safety to the workmen and the public. 10. All forms shall be carefully checked for alignment and plumb before concrete is placed. Contractor shall notify the Architect when formwork is in place and ready, and shall allow the Architect three (3) days to inspect and approve the accuracy of the form placement before pouring concrete. 11. Concrete must be smooth and monolithic and must be free of voids, spalled areas, loose aggregate, and sharp protrusions, with no coarse aggregate visible. 12. Concrete may be placed with the temperature below forty(40) degrees F. by heating the materials and protecting the fresh concrete from the cold weather as specifically directed by the Architect. Page - 4 CEPHAS RESIDENCE October 25, 1996 Fairweather-Brown/Architects & Designers 13. Anchor bolts shall be furnished and set in all exterior foundation walls as indicated on the drawings. Unless otherwise noted, they shall be 1,12" diameter x 18" long, placed 6'- 0" on center (maximum), with a minimum of two (2) anchor bolts per sill plate. 14. Prior to placing vapor barrier, surfaces shall be smoothed to prevent protrusions which may cause damage to the barrier. Metal form ties to be snapped off flush with walls inside and out and all holes to be pointed. 15. Liquid Membrane: See TFIERMAL AND MOISTURE PROTECTION 16. Sheet Membrane: See THERMAL AND MOISTURE PROTECTION DIVISION 4 : MASONRY GENERAL: Provide all labor, materials, transportation, equipment and services necessary to complete all masonry work required by the drawings as specified herein, or reasonable implied as necessary to complete the work. NIORTAR Scope: Mortar for all unit masonry. 1. Mortar shall not be mixed when atmospheric temperature is below forty (40) degrees F., unless special precautions approved by the Architect are taken. 2. Mortar not placed within two (2) hours of mixing shall be discarded. 3. Mixing shall be by machine only. Changing source or brands of materials during course work is prohibited unless approved by the Architect. 4. Portland cement shall conform to ASTM C150, Type I. 5. Non-staining Cement shall be used in contact with cut stone and within twelve (12) inches of same. Non-staining Cement shall be not more than .03% soluble alkali as per Federal spec. SS-C-18 le. Use white sand for non-staining mortar. 6. Hydrated lime shall conform to ASTM C207 Type S. 7. Quicklime shall conform to AS TNI C5. 8. Sand shall be natural, clean, washed, hard particles of mineral origin containing less than .06%chlorides, otherwise conforming to ASTM C144. 9. Water shall be potable. Mortar type M (ASTM C270) for masonry in contact with earth and below grade shall be composed of one (1) part Portland cement, 1/4 part lime, three (3) parts sand. 11. Mortar Type S (ASTM C270) for exterior masonry shall be composed of one (1) part Portland cement, 1/4 part lime, four(4)parts sand. 12. Pointing mortar (exterior masonry) shall be one (1) part Portland cement, 1/10 part lime, two (2) parts sand. Pointing mortar shall conform in color and component material to setting mortar. 13. Grout shall be one (1) part Portland cement, one (1) part aggregate. For thicknesses under one (1) inch use size No.1 aggregate; over one (1) inch use size No. 8 aggregate as per ASTM C404. UNIT MASONRY Scope: This includes but is not limited to: A. Concrete Blockwork B. Brickwork C. Brick Pavers 1. Cast Iron Drainage Piping (ANSI A40.5) where required by code shall consist of extra-heavy, uncoated cast iron soil pipe and fittings. h. FLASHING MATERIAL shall be sheet lead, weighing not less than 4 pounds per square foot. All lead shall be 99.5% pure. i. FITTING MATERIALS shall conform to the type of piping material used. j. UNIONS shall have metal ground joint seats. hi copper piping or copper tubing 2" or smaller, unions shall be cast bronze 150 lb. class with bronze to bronze seats and with screw ends, brazing ends, solder ends, or with adapters, as required. All other unions shall be cast flanged unions of materials and of working pressure to correspond with pipe. k. VALVES shall be first quality, free from all imperfections and defects. All valve parts of same manufacture, size and type shall be interchangeable. All hand operated valves shall be designed to open in a counter-clockwise direction. Piping work shall be coordinated so that one inch minimum free space will result around all insulated piping after the insulation is applied. 11. All piping shall be hung or supported by approved hangers, supports, anchors, sway braces, guides and special hangers as specified herein. One pipe shall not be hung or supported from another. 12. Contractor shall test the Plumbing system before final acceptance by the Owner. All appropriate operational and maintenance manuals shall be provided. 13. Coordinate all work with other trades prior to roughing-in and equipment installation.
