inspector with the following: for new building or new use:
!. Final survey of property w±ch accurate location of all buildings, property lines,
s~reecs, and unusug! nature! or topographic features.
2. Final Approval from Health Sept. of water suppiy and sewerage-disposa!(S-9 form).
3. Approval of eleccrica! insca!iacion from Board of Fire Unde~rriters.
4. Sworn statemen~ from plumber certifying thee the solder used in syscem contains
less ghat 2/10 of 1% lead.
5. Commercial building~ industrial building, mu!ciple residences and sL~i!ar buildings
and insta!la~ions, a cerclfmcate of Code Compliance from architect or engineer
reeponsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
S. For existing suildings (prior no April 9, i957) non-conforming uses, or buildings and

Please be advised that I inspected the instillation of the two footings for the referenced
permit. They were concrete, 16 inches in diameter 2 feet thick and the bottom of the footing was
3 feet below the grade.
