APPLWAATION FOR CERTIFICATE OF OCCUPANCY
A. This application must be filled in by typewriter OR ink and submitted to the buil,
inspector with the following: for new building or new use:
I. Final survey of property with accurate location of all buildings, property lir
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 for
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contain
less than 2/10 of 12 lead.
5. Commercial building, industrial building, multiple residences and similar buil
and installations, a certificate of Code Compliance from architect or engineer
responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings
'-'pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and

Dear Mr. Fish
Your office is now in receipt of my application for a building permit for the enlargement
of the shed on my property. I waited to file the application until the shed was moved so
that I could accurately indicate the setback on the application. That work was completed
yesterday.
As per your directions,the relevant monuments are exposed, staked and attached by
string.
The shed now meets setback requirements as follows:
The north corner nearest the road when measured in a perpendicular line to the
property boundary is set back 48 feet
The south corner nearest the road when measured in a perpendicular line to the
property boundary is set back 44 feet. l
Because the property is at an angle,the shortest distance from the shed to the i
property boundary is also at an angle from the south corner to the boundary. 11at fI
distance is 41.5 feet.
The setback from my neighbor's property on the north side is 6 feet 2 inches.
If it is possible given your busy schedule,I would request an inspection prior to
November I lth. From November 1 I through November 23 I am required to be out of
town on business and would like to be present at the inspection.
