Dear Permittee:
Your recent request to modify the above permit has been reviewed pursuant to 6NYCRR, Part
621. It has been determined that the proposed modifications will not substantially change the
scope of the permitted actions or the existing permit conditions. Therefore, the permit is amended
to authorize:
The enclosure of a portion of the approved enlarged deck with a glassed and screened patio
enclosure (I 5'xl 8').
This letter is an amendment to the original permit and, as such, shall be available at the job site
whenever authorized work is in progress.
All other terms and conditions remain as written in the original permit.

Dear Mr. Boufis:
I received the above Building Permit to enlarge my deck. At that time, I also
requested that the permit allow me to add a glass and screen Patio room on top of
part of the deck. I was not aware that I needed permission from the DEC and the
Southold Town Board of Trustees to do so. The enclosure will measure 15'x18' as
per the enclosed drawings. It will be for seasonal use and will not have any
plumbing. I now have an amendment to my DEC permit and a Waiver from the
Southold Town Board of Trustees (copies attached) to add the enclosure.
I now request an amendment to my Building Permit to allow me to have the Patio
room constructed. The deck is scheduled for inspection on Wednesday, September
9, 1998.
As required, I have also attached three (3) copies of the builder's drawings of the
room, a picture of the new deck, and the brochure from the company that
manufactures the structure further describing the construction.
