I conducted all required inspections regarding footings, rebars, anchor bolts and dampproofing. The contractor decided to install a crawl space between the existing dwelling and the proposed addition as opposed to bridging over as depicted in the permit. He consulted with me before making the change. Please refer to the attached amended foundation plan. I hereby certify—to the best of my professional knowledge-that the work was performed in compliance with all N.Y. State and local rules and regulations and in accordance with the architect's plans and specifications including the revised crawl space.
