A. This application must be filled in by typewriter OR ink and submitted to the building
inspector with the following: for new building or new use:
1. Final survey of property with accurate location of all buildings, property lines,
streets, and unusual natural or topographic features.
2. Final Approval. from Health Dept. of water supply and sewerage-disposal(S-9 form) .
3. Approval of electrical installation from Board of` Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildin:
and installations, a certificate of Code Compliance from architect or engineer
responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.

Occupancy was never issued for the deck addition that you
added to your house under the above building permit .
The fee was paid , but a Certificate of Occupancy was
never issued because an Underwriters Certificate was never
received .
Kindly submit the Underwriters Certificate and we will
issue the Certificate of Occupancy .
Occupancy or use is unlawful without a Certificate of
YOccupancy . Please clear up this matter as soon as possible so
that legal action does not have to be taken .

3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Nonconforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

Dear Mr. Cichanowicz:
During a review of our files, it was noted that the above
building permit has expired and a Certificate of Occupancy has
not been issued.
According to the Code of the Town of Southold Article XIV,
100-141 and 100-144 a building permit is needed for construction
of a deck addition to an existing dwelling. It is unlawful to
occupy a dwelling until a Certificate of Occupancy has been
issued.
The final inspection has been made and application and
payment for the Certificate of Occupancy was received 9/9/80.
Please submit the Underwriters Certificate so that we may issued
said Certificate.
This is your third notification. In order to avoid any
legal action you must correct this violation immediately.
