Approval of electrical installation from Board o{ Fire Underwriters.
Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of 1% lead.
Commercial building, industrial building, multiple residences and similar buildings

Dear Mr. Boufis:
Please be advised that I am the New York State Professional that has/had been
retained by Gusmar Realty Corp., located at 13-15 37th Avenue, Long Island City,
New York, to observe, document, and modify the inadequate and improper framing at
the afore to described property.
As you are aware, the initial framing inspection, which was performed by your
Department, revealed numerous conditions which were considered inadequate,
precarious, and not in keeping with framing standards. These inadequate and
negative conditions were apparently causative to the Building Department not having
passed the mandatory framing inspection at this location. Subsequent to this, Mr.
Fakaris of Gusmar Realty had requested that Building System Evaluations and
Related Services visit the site, perform an inspection, and identify the readily visible
inadequate, precarious, and negative conditions. These requested inspections were
performed on November 16, 1999 and again on November 17, 1999. As part of this
inspection procedure, we had met with the Contractor's Superintendent, Mr. D.
Dragonetti, and reviewed the required framing corrections with him. This was
accomplished and the framing (sub) Contractor corrected the inadequate, improper,
and negative conditions. These modifications were identified on the revised (existing)
framing plans which was prepared by the undersigned and issued to the (sub)
Contractor.

Subsequent to the last site visit of November 17, 1999 an additional site review was
performed by my office on December 17, 1999. Based upon this last site review, it
would now appear that the significant non conforming precarious and inadequate
conditions have now been ameliorated and that the framing is now adequate for its
intended purpose. Please note that the Contractor/Owner (Gusmar Realty Corp.) has
been advised that as per the December 17, 1999 site visit, that only miscellaneous
work items including wood blocking and additional center nailings are requested. The
Contractor's Superintendent, Mr. Dragonetti, has assured us that these few minor
conditions will be eliminated by the end of this week. This will complete all of the
required corrective work and place the premises (to date) In Compliance with the
Code.
In closing, should you have any questions pertaining to this matter, or require any
additional information, please do not hesitate to contact me accordingly. In the interim,
