APPLICATION FOR CERTIFICATE OF OCCUPANCY
This application must be filled in by typewriter OR ink and submitted to the building
inspector with the following: for new building or new use:
1. Final survey of property with accurate location of all buildings, property lines,
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board o{ Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildinl
and installations, a certificate of Code Compliance from architect or engineer
responsible for the building.

EnVironmentat Conservation has determined that. the property landward of the functional
bulkhead.greater than 100 ft. in length, constructed prior to August 20, 1977 as shown on ·
aerial photograph #72-2024 dated April 6, 1976 is beyond the jurisdiction of A~lJc~e 25 Tidal
Wetlands.
Therefore, in accordance with the current Tidal Wetlands Land Use Regulations (6NYCRR
Part 661) no permit is required under the Tidal Wetlands ACt. Please be advised, however,
that no construction, sedimentation, or disturbance of any kind may take place seaward of the
tidal wetlands jurisdictional boundary, as indicated above, without a permit. It is your
responsibility to ensure that ail necessary precautions are taken to prevent any sedimentation
or other alteration or disturbance to the ground surface or vegetation within Tk~al Wetlands
jurisdiction which may result from your project. Such precautions may include maintaining
adequate work area between the tidal wetland jurisdictional boundary and your project (La. a
15' tO 20' wide construction area) or erecting a temporary fence, barrier, or hay bale berm.
Please be further advised that this letter does not relieve you of the responsibility of obtaining
any necessary permits or approvals from other agencies.

The following action was taken by the Board of Town Trustees
d~ring its regular meeting held on Apri! 21, 1999 regarding the
above matter:
WHEREAS, Inter-Science Research Assoc. Inc. on behalf of PATRICK
KELLY applled to the Southold Town Trustees for a permit under
the provisions of the Wetland Ordinance of the Town of Southold,
application dated March 18, 1999 and,
WHEREAS, said application was referred to the Southold Town
ConServation Advisory Council for their findings and
recommendations, and,
WHEREAS, a Public Hearing was held by =he Town Trustees with
respect to ~aid application on April 21, 1999, at which time all
inte=ested per~ons were given an opportunity to be heard, and,
WHEREAS, the Board. members have personally viewed and are
familiar with the premises in question and the surrounding area,
and,
WHEREAS, the Board has considered all the testimony and
documentation submitted concerning this application, and,
WHEREAS, the structure complies with the standard set forth in
Chapter 37-18 of the Southold Town Code,

NOW THEREFORE BE IT,
RESOLVED, that the Board of Trustees grant a Wetland Permit to
PATRICK KELLY to construct a 20'X 40' in-ground swimming pool
approx. 50' from the bulkhead and a 1300 s.f. brick and sand
at-grade patio located approx. 45' from the bulkhead with the
condition that the'buffer be maintained as it is to date, and
drywells be installed for the backwash of the pool.

during its regular meeting held on ApriI 21, 1999 regarding the
above matter:
WMEREAS, Inter-Science Research Assoc. I~c. on behalf of PATRICK
KELLY applied to the Southold Town Trustees for a perm/t under
the provisions of the Wetland Ordinance of the Town of Southold,
application dated March 18, 1999 and,
WHEREAS, said application was referred to the Southold Town
Conservation Advisory Council for their findings and
recoma~endations, and,
W~EREAS, a Public Hearing was held by the Town Trustees with
respect to ~aid application on April 21, 1999, at which time all
interested persons were given an opportunity to be heard, and,
WHEREAS, the Board members have personally viewed and are
familiar with the premises in question and the surrounding area,

WHEREAS, the Board has considered all the testimony and
documentation submitted concerning this application, end,
WHEREAS, the strl/cture complies with the standard set forth in
Chapter 37-18 of the Southold Town Code,

NOW THEREFORE BE IT,
RESOLVED, that the Board of Trustees grant a Wetland Permit to
PATRICK KELLY to construct a 20'X 40' in-ground swimming pool
approx. 50' from the bulkhead and a 1300 s.f. brick and sand
at-grade patio located approx. 45' from the bulkhead with the
condition that the'buffer be maintained as it is to date, and
drywells be installed for the backwash of the pool.
'BE IT FURTHER RESOLVED that this determination should not be

or similar project.
Permit to construct and complete project will expire two years
from the date. it is signed. Fees must be paid, if applicable,

Therefore, in accordance with the current Tidal Wetlands Land Use Regulations (6NYCRR
Part 661) no permit is required under the Tidal Wetlands Act. Please be advised, however,
that no construction, sedimentation, or disturbance of any kind may take place seaward of the
tidal wetlands jurisdictional boundary, as indicated above, without a permit. It is your
responsibility to ensure that all necessary precautions are taken to prevent any sedimentation
or other alteration or disturbance to the ground surface or vegetation within Tidal Wetlands
jurisdiction which may result f~om your project. Such precautions may include maintaining
adequate work area between the tidal wetland jurisdictional boundary and your project (/.e. a
15' to 20' wide construction area) or erecting a temporary fence, ban'ier, o~ hay bale berm.
