The undersigned petitioner, ANN P. KWASNESKL residing at (no number
North Parish Drive, Southold, New York, liable to be assessed for highway
taxes in the Town of Southold, hereby apply to the Town Superintendent of
Highways to lay out new highways and drainage area at Southold, in the Town
of Southold, County of Suffolk and State of New York as follows:
1. The highway is known as SLEEPY HOLLOW LANE, as shown and
designated on a certain map entitled, "Sleepy Hollow" and filed in the Office
of the Clerk of the County of Suffolk on February 4, 1976, as Map No. 6351,
Abstract Number 8002.
2. The highway known as GRISSOM LANE, as shown and designated on a
~ertain map entitled, "Sleepy Hollow" and filed in the Office of the Clerk of

4. The highway known as WILLOW POND LANE, as shown and designated

I, ANN P. KWASNESKI, residing at (no number) North Parish Drive,
Southold, New York, being the owner of certain land in Southold, Town of
Southold, Suffolk County, New York, and included within which land are pro-
posed highways known as SLEEPY HOLLOW LANE, GRISSOM LANE, and
WILLOV~ POND LANE, described in the application in the above entitled
matter dated
NOW THEREFORE, in consideration of ONE ($1.00) Dollar to us in hand
paid by Raymond C. Dean, residing at Peconic Lane, Peconic, New York,
11958, Superintend'ent of Highways of said Town of Southold, and in further
consideration of laying out of the new highways known as SLEEPY HOLLOW
LANE, GRISSOM LANE, and WILLOW POND LANE, do hereby release and
dedicate to the Town of Southold all of the land heretofore owned by me
within said proposed highways as follows:
1. The highway known as SLEEPY HOLLOW LANE, as shown and desig-
nated on a certain mape entitled, '~leepy Holow" and filed in the Office of
the Clerk of the County of Suffolk on February 4, 1976, as Map No. 6351,
Abstract No. 8002.
2. The highway known as GRISSOM LANE, as shown and designated on a
certain map entitled, "Sleepy Hollow" and filed in the Office of the Clerk of
the County of Suffolk on February 4, 1976, as Map No. 6351, Abstract No. 89
4. The highway known as WILLOW POND LANE, as shown and designated
on a certain map entitled, "Sleepy Hollow" and filed in the Office of the
Clerk of the County of Suffolk on February 4, 1976, as Map No. 6351,
Abstract No. 8002.
And I do release the said Town of Southold from all damages by reasons
of the laying out of the said highways.

Southold, in the County of Suffolk and State of New York, having duly met at
a regular meeting of the said Town Board on the 23 day of Nove~aber , 1982,
and having duly considered the application o~ ANN P. KWASNESKI, dated the
10flu day of September, 1982, for laying out of new highways at Southold, Town
of Southold, to be known as SLEEPY HOLLOW LANE, GRISSOM LANE, and
WILLOW POND LANE, as Southold, Town of Southold, do hereby consent that
such laying out be made in accordance with said petition.

WRITTEN application having been duly made to me, the Town Superin-
tendent of Highways for the Town of Southold, Suffolk County, New York, for
the laying out of Town Highways in the said Town of Southold, by ANN P.
KWASNESKI, liable to be assessed for highway taxes in the said Town, and
a dedication and release from damage having been executed, acknowledged and
delivered by the owner of the land through which the proposed highways are
proposed to be laid out, a copy of which is annexed hereto and nothing having
been paid to any claim for such damages:
NOW THEREFORE, I, the Town Superintendent of Highways of the Town
of Southold, Suffolk County, New York, DO HEREBY DETERMINE AND ORDER
that the town highways are hereby laid out at Southold in the Town of Southold
as follows:

Dear Judy:
At long last, all of the dedication questions relative to the above have
been taken care of and I hand you herewith the dedication papers con-
sisting of the Application, Dedication and Release, Consent of the Town
Board and Order of the Superintendent of Highways. Also enclosed are
two releases of part of mortgaged premises as well as a map showing
the streets in question and a letter from the Assessors indicating that
the improvements satisfy the requirements of the regulations.
I also have in my office the dedication papers for Greton Estates and
Eastwood manner. However, further documents and unresolved
questions must be concluded before these are ready for Town Board
action.

Dear Gary:
I have received the subdivision map of the above matter and have the following
comments:
(1) Grissom Lane deadends at its southerly terminus. Under the highway
specifications, a 100 foot by 100 foot dedicated turnaround must be provided.
(2) The subdivision map indicates a street known as Willow Pond Lane
running northerly from the westerly terminus of Sleepy Hollow Lane.
Willow Pond Lane is not included in the dedication. If this street was
omitted by oversight, the papers must be amended to include it. On the
other hand, if Willow Pond Lane is not to be dedicated, then the westerly
terminus of Sleepy Hollow Lane would deadend at land of Dart and accord-
ingly a 100 foot square turnaround must be provided.
(3) The title report does not indicate that a tax search was made. A tax
search is required.
