This Firm represents the owner of the subject property, who is seeking to obtain all
required regulatory approvals to enclose the existing seaward deck thereon as well as to
install two (2) bay windows along the landward face of the existing dwelling. Prior to
making a formal application to your Board, we would like to meet on-site to discuss
intended site improvements. Consequently, attached herewith please find the following;

(3) Owner's Endorsement, (one [1] original);
(4) Owner's Open Government Disclosure Form, (one [1] original);
