APPLICATION FOR CERTIFICATE OF OCCUPANCY
This application must be filled in by typewriter OR ink and submitted to the building
inspector with the following: for new building or new use:
1. Final survey of property with accurate location of all buildings, property lines,
streets, and unusual natural or gopographic features.
2. Final Approval from Health Dept. of wager supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Bworn statement from plumber certifying that the solder used in system contains
less than 2/10 of 1% lead.
5. Commercial building, indusgrial building, multiple residences and similar buildings
and imstallattons, a certificate of Code Compliance from architect or engineer
responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and

1. Accurate survey of property showing all propergy lines, streets, building and
unusual natural or topographic features.
2. A properly completed application and a consent to inspect signed by the applicant.
If a Certificate of Occupancy is denied, the Building Inspector shall state the
reasons therefor in writing to the applicant.

Re: Union Chapel Accessible Bathroom
Dear Mr. Semone,
As per your request here is a summary of the non-compliance issues along with
citations to aid in understanding our vadance request to the state to construct an
accessible bathroom in our existing church:
The problem is that our bathroom has to be 58" wide rather than 60". It lies between an
egress hallway that can't be narrowed by and a furnace that cannot be moved.
1. Wheelchair Turning Space: is reduced to 58" rather than the 60" required under
(4.2.3) by the walls.
2. Wheelchair Turning Space: is reduced 55' rather than the 60" required under
(4.2,3) by the grab bar adjacent to the water closet. It is not clear to me whether
a grab bar is an obstruction.
3. Location of the Water Closet: The distance from the side wall to the canterline
of the water closet should be 18" {4.17.1) but 16 "is shown. We did this because
we thought it was more important to obtain the maximum Wheelchair Turning
Radius Fig. B4.2.3.
Wheelchair Accessible Toilet Stalls - Door Swin,q In: As per Fig. B4.18.3.2 a
60" minimum clear floor space is required. Our space is only 58'
Clear Space at Lavatory: is reduced to 26" x 48" rather than the 30" x 48"
required under (4.20.3.2). This is because the door swings into the clear lavatory
space.
Size of Entry Door: The 2'10" door specified will yield a 2'-71/2" opening when
opened to 90 degrees and does not meet the 2'-8" required clear opening under
4.13.6.1 {Fig. B4.13.5 (a)) We thought that a larger door would bump into the
lavatory. Also we thought the door itself would create an obstruction in the space
as the wheel chair bound person would have to wheel further away to escape its
swing.
Let me know of any further problems you notice so that our variance is a thorough as it

c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on thc premises available for inspection throughout the work·
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
fl Every building perrmt shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting th~
property have been enacted in the interim, the Building Inspector may authorize, in writing, the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.
