I, Thc)mas M. Ludlow, as Contractor and authorized agent, propose an addition and renovations to
the existing single family residence located at. 3190 Park avenue, Mattituck, NY, 11952. The
Suffolk County.' tax number is 1000,123-08-20. The owners are James & Carol Mikelbank, 3190
Park Avenue, Mattitnck, NY 1 !952 ( phone # 516-298-5745). The proposal includes a one story..'
add/t/on of approximately 384 sq. fi. (footprint) on the northeast side of the residence and an
addition to an existing wood deck of approximately 100sq. fi. on grade on the north east side of the
residence. There is r~o proposed construction seaward of the existing structure. The deck shall lie
w/thin sixty feet of the existing bulkhead and approximately one hundred and m,enty feet ti'om the
hi~ tide mark. The addition shall lie withh~ seventy feet of the existing bulkhead and approximately
one hundred and thirty feet from the h{~ fide mark. No si~ificant change in grade, removing of fill
or adding of fill as proposed. :

as agent, on the owners behalf and two site plans drawn by the Mike!banks architect, Mr. Donald G.

Dear Mr. Ludlow,
The Southold Town Board of Trustees reviewed .the survey dated
and receiVed in our office on August 12, 1998 and determined
that your proposal to construct aconstruct a Master Suite,
replace windows, roof shingles, siding and enlarge rear deck, to
be out of the Trustee Wetland jurisdiction under Chapter 97 of
the Town Code.
However, any activity within 75' of the Wetland line requires a
permit from this office.
This determination is not a determination from any other agency.
