Ludlow, as Contractor and authorized agenl, propose an addition and renovations to the ex/sting single family residence located at 3190 Park avenue, Martituck, NY, 11952. The Suffolk Coun¢' tax number is 1000-123-08-20. The owners are James & Carol Mikelbank, 3190 Park Avenue, Mattituck, NY 11952 ( phone # 516-298-5745). The proposal includes a one sto~' addition of approximately 384 sq. fi. (footprint) on the northeast side of the residence and an addition to an existing wood deck of approximately 100sq. fi. on grade on the north east side of the residence. There is no proposed construction seaward of the existing structure. The deck shall lie within sixty feet of the existing bulkhead and approximately one hundred and m~enty feet t?om the hi~ tide mark. The addition shall lie within seventy feet of the existing bulkhead and approximately one hundred and thirty feet from the hi.~a fide mark. No significant change in grade, removing of fill or adding of fill as proposed. However, any activity within 75' of the Wetland line requires a permit from this office. This determination is not a determination from any other agency.
