Instructions
A. This application must be filled in typewriter OR ink, and submitted ia Illll~l~ to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2.Final approval of Health Dept. of water supply and sewerage di~posal-(S-9 form or equal).
3.Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5.Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of p~operty showing all property lines, streets, buildings and unusual natural or
topographic features.

HOME INSPECTION

GENERAL:
The house is a single family, single story, wood frame
construction with full basement and crawl type attic space
accessible from the attached garage area by ladder.
The home is unoccupied and has been vandalized to a point
that complete renovation of the interior is required. All
utilities have been vandalized beyond repair.
Structural members and exterior roof and siding appear to be
in fair to Good cond{tion.
The home has three
dinette, full bath,
square feet.

living room, kitchen
area of approximately 1,200
STRUCTURAL:
Floor joists and all structural members appear to be in good
condition.
Basement foundation wall requires reinforcement in several
locations and re-painting of some masonry units is required.
Stoops and stairs at entrance or exit doors require
replacement, also masonry stairway from dinette to garage
needs to be renewed.
Visual inspection showed no signs of rodent or termite
infestation.

EXTERIOR~
Asphalt shingle roof appears to be in fair condition.
All windows and doors except the front entrance door have
been vandalized, therefore, all must be replaced with new.
The exterior shake siding appears in fair condition, and
with minor repair along with two (2) coats of stain, the
siding should last many years.
Regrading of the area will be required to prevent rain water
from settling next to the foundation walls. New driveways
and walks are also required for, at present, they are non-
eMistant. EMit of the dinette area requires a deck and
stairs to ground level.
INTERIOR~
The hard wood flooring throughout the interior of the home
requires refinishing.
All plasterboard walls need repainting and, in many areas,
minor patching, spackling, etc. is required.
All bedroom closet doors need replacing as well as all
interior doors.
The bathroom has been damaged by vandals to a point where
the entire walls, floors, and fixtures require replacement.
The front door requires re-finishing and new hardware which
should restore it to a new condition.
New windows and doors are required in all rooms and new
kitchen cabinets, appliances, and tileing in the kitchen
dinette area.
PLUMBING~
Whereas all plumbing has been damaged or removed, the entire
home will require plumbing throughout. This will include
the sanitary sewage system and potable water system.

HEATING:
The entire heating system has been removed which includes
heating boiler, baseboard radiation system, and all
componant parts. The fuel oil tank is corroded and damaged
and would require pressure tests, cleaning, and perhaps
repair, but due to its condition and age, a new tank would
be more economical.
ELECTRICAL:
Electrical services to the home are non-existant. New
e~te~ior feeders, meter bo~, etc., plus new interior
distribution panel of no less than 15~ AMP must be
installed. All electric wiring and and room outlets must be
rechecked for safety purposes and Code requirements.
ENERGY CODE: %/~
Walls are insulated with two <(~)~ inches of batt insulation.
~eiling would require addttio~a'~ insulation to be in
compliance with the State Energy Code. Caulking and
sealing, especially in the basement, in also required.

sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets
or areas, and giving a detailed description of layout of property must be drawn on the diagram which is part of this appli-
cation.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issued a Building Permit t6 the applicant. Such permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose whatever until a Certificate of Occupancy
shall have been granted by the Building Inspector.
